responsible for the quality and correctness of the tests/results. HealthCheck Thyrovouchers are valid for six
months from the date of Issuance and no refund/extension is allowed in case of non utilisation of the
facility/service within that period. Thyrovoucher is invalid without barcode number. Thyrovoucher cannot be
exchanged in lieu of cash by the member or the Beneficiary. Any disputes arising out of this HealthCheck Profile
to be dealt by the Beneficiary directly with the Thyrocare Technologies Limited, Plot No.D-37/1, TTC MIDC,
Turbhe, Navi Mumbai - 400 705 and will be referred to a sole arbitrator under Arbitration and Conciliation Act,
1996 as amended from time to time. The courts in Mumbai will have jurisdiction to entertain, try and dispose
off any disputes/ suits/ claims arising out of this.
