CAROUSEL PLAYSCHOOL

ENROLLMENT AGREEMENT
YEAR 2010

Because Carousel Playschool is a cooperative and non-profit organization, its


success depends on true cooperation among the participating parents. This puts a
great responsibility on us all, and each parent must do his/her share.

Any suggestions for the betterment of the school and its programming are always
welcome. The quality of our school depends on every members participation.

As a member of Carousel Playschool, I realize and accept my responsibilities to the


school. These include:

1. Parent-helping as required.
2. Attendance at Orientation sessions.
3. ACTIVE membership on a committee or board position.
4. Attendance at Carousel functions: Fall Family Potluck, Winter Holiday
Program, Trike-a-Thon, Annual Garage Sale, and Graduation.
5. Participation in cleaning and maintaining the facilities.
6. Drive for a field trip.
7. Timely payment of registration and tuition fees.
8. Meeting fundraising obligations within specified time frames either by
participation in fundraising activities or other means.
9. Working a minimum of 1 hour during set-up, sale hours, or clean-up of our
annual Garage Sale.

I have received and will read/comply with the rules set forth in the Parent
Handbook. I also understand that these responsibilities are listed beginning on
page 5 of my handbook.
