Dear Students,

You've received notifications about the availability of the Flu vaccination, which includes H1N1.  ALL
College of Medicine students are required to obtain this vaccination and provide the Office of Student
Affairs with a receipt/documentation of this vaccination.  This is a yearly requirement and providing a
copy to us insures that we know that you are in compliance.  Please be sure to write your name and UIN
on your receipt/documentation.

This is only one of many vaccinations we, the Office of Student Affairs, will remind you is needed in
order for you to participate in your clinical course work/rotations.  M1's and M2's participate in clinical
course work in ECM.  M3's and M4's are on clinical rotations.  The Hospitals that you report to require
our office, and you, to provide them with documentation of your immunization and administrative
document compliance.  Our office is here to assist with this process.

Please look out for emails from either myself, LeAmye Howard, or Monica Mason-Hayden.  We
appreciate your timely response to these requests.
