 I. INTRODUCTION

The reason of existence in every faces of communities came by its differences.

Its differences came by the influences of each individuals and families. The families are
the major structure to produce a certain community. How does a family give an impact
to the society? What are their functions in contribution to the improvements of health?

Individual alone cannot do the task of improving the health situation in the
community. To ensure success, the collective effort of the people in the community, the
local government and the national government must be serious in ways of
implementation. The interest of people is the first factor in having successful
implementation of rendering health services. There acceptance of the new knowledge
may help in easy embracement of the purpose. Second factor of success is how serious
the Local and National Government wanted to extend the hands to the people. Because
of there unwilling reach; they may just neglect the improvements of health education in
the community. And last is the community health workers and trained personnel that
connects the aim and goal of the Government and the needs and shout of the people.
By these people, they may be able to gather the shout of the people and may extend in
to the local Government.

Community health nursing practice promoters and preserves the health of

populations by integrating skills and knowledge relevant to both nursing and public
health. It is comprehensive and general and it is not limited to a particular age or
diagnostic groups. It is continual and is not limited to episodic care, while the community
health nursing practice includes nursing directed to individuals, families and groups, the
dominant responsibility is to the population as a whole.

A. OBJECTIVES OF THE STUDY

This case study had conducted to implement the following: First is to survey the
potential outlook of the community to improve there health status. Second, is to identify
problems related to health and environment. Third is able to apply community Health
