ISSUE OF BIRTH CERTIFICATE WITH NAME

Dear Sir,

1. Our son was born at Military Hospital on 19/11/1995. It was recorded and Birth
Certificate was issued vide National Capital Territory of Delhi, Deihi
Cantonment Sub-Registrar Office Certificate of Birth Sl No 44275, Registration
No 287 with Date of Registration on 29/12/95 (Copy attached for ready
reference).

2. Now a days, various academic institutions/Govt authorities ask the birth

certificate with name mentioned in the birth certificate. Hence, it becomes
mandatory to have the birth certificate with the child name written on it.

3. .In view of the above, I request you to issue a ‘Certificate of Birth’ with my
son’s name ( SABHARI NATARAJAN ) written on it. I have enclosed a self
addressed & stamped envelope for dispatching the same to me. I have also
enclosed a copy of son’s mark sheet for your information.
