• Full payment is due upon registration. If you withdraw from the tournament you will be
charged a $200 penalty fee and have the balance of your entry fee returned.
• All teams must provide an official signed and certified USA Hockey roster by November
22, 2010.
• Teams from outside of Illinois must provide a travel permit
