Good morning sir,

This is anil from vizag branch, regarding leaves I have taken this month.

The reasons for particular dates are:

1. 04/10/2010 I went to my MCA College regarding fee payment issues.

2. 15/10/2010 I have taken half-day leave to attend my cousin’s marriage in my
home town.

3. 16/10/2010 & 18/10/2010 while returning from marriage unfortunately we had an

accident that night. Because of that I was unable to attend the
office.

17/10/2010 was Sunday, because I was failed to attend before and after Sunday. It was
also counted.

For all the leaves I kept mail and given Hard Copy of leave letter to JayaLakshmi Madam.
