Pursuant to the state open records act, I request access to and copies of any and all of the following

(A) EBRSO Policy and Procedures Handbook.

(B) Pertaining to Deputy Joseph Lochridge (#1671):

(1) Reports of internal investigations.

(2) Records of Attendance.

(3) Annual leave slips.

(4) Sick leave slips and absence records.

(5) Appointment affidavits

(6) Record of appointments

(7) Record of changes in status or position (promotion, reassignment, etc.)

(8) Copy of current position description

(9) Records of completed training courses

(10) Letters supporting formal disciplinary actions

(11) Pre-discipline notices

If any records in my request are denied in whole or part, I ask that you justify all deletions by reference to
specific exemptions of the act.
