On Tuesday September 21 I was sent to Bottling 3 to load 8 pallets of Southern Comfort onto a Louisville

Cartage truck that had arrived at the outside loading ramp to pick up the load. I went over to the
Bottling 3 warehouse to get the details about the loading process and was told that they would wrap
and send the pallets down per freight elevator and I would load them on the truck from there.

Pam Combs accompanied the pallets as they came down, and told me to go ahead and get the pallet
from the elevator. I did not know that the elevator would not support the forklift, and when I started in
it immediately sank below the surface level about 8-12 inches. I sat the brake, turned the forklift off,
exited and called security for further assistance.

After the fact, Pam Combs said that she had forgotten that the elevator would not support the forklifts,
and that she had had that happen once before. Chuck Combs arrived to assess the situation, and called
maintenance to assist. Maintenance determined that a 4 wheel drive truck and a tow strap would be
the best approach, and arrived immediately with the resolve. A strap was hooked to the forklift and it
was removed (by maintenance) without further incident. The 4 wheel drive pickup truck pulled the
forklift back onto the ledge in seconds. Nothing was damaged nor was anybody injured.

Afterwards, when Pam brought the pallets down on the elevator, she made sure they were at the edge
of the elevator so they could be taken off from outside the elevator without having to enter it. We
continued this process until the 8 pallets were successfully loaded on the truck.
