We, Group 39 of BSN 10-39 of Angeles University Foundation are currently
accomplishing our case study for in partial fulfilment of the requirements in Nursing
Care Management 105 RLE.

In this regard, we would like to ask a permission from you to allow us to do a

records review of Juan Calbang’s chart. He has an admitting diagnosis of “to
consider Septic Shock secondary to Hospital Acquired Pneumonia, Cerebrovascular
accident right thalamic midbrain, Chronic kidney disease secondary to hypertensive
nephrosclerosis” with the case no. 10-1354-B. He was admitted on November 10,
2010 and discharged on November 22, 2010 id to collect data about patient’s case.

Whatever information obtained will be used only for the above mentioned
purpose and confidentiality of data will be maintained.
