Adenocarcinoma of the rectum arises as an intramucosal epithelial lesion, usually in an adenomatous polyp or
gland. As cancers grow, they invade the muscularis mucosa, lymphatic structures, and vascular structures and
involve regional lymph nodes, adjacent structures, and distant sites, especially the liver.
