Role of Vitamin D
• Promote mineralization of new bone
• Increase serum Ca++
• Increase serum Phosphate concentration
Synthesis of Vitamin D
Action of Vitamin D
• Intestine: Active form of VitD – 1,25-
dihydroxycholecalcifer induces synthesis of
Calbindin D-28K. This protein binds to Ca++
thus promoting intestinal absorption.
• Kidney: Similar MOA to Intestine in stimulating
both Ca++ & also stimulate Phosphate
reabsorption.
• Bone: Active form act with PTH to increase
osteoCLAST activity & bone resorption. This
allows to resorb the old bone and provide
more Ca++ available to mineralized new bone.
Pathophysiology of Vitamin D
• Rickets/Osteomalacia: Insufficient amounts of
Vitamin D causes deficiency in Ca++ &
phosphate available to mineralize growing
bones. Result in bending/softening of bones.
• Vitamin D resistances: kidney is unable to
produce the active metabolite. Usually caused
by an absence of 1a-hydroxylase or by chronic
renal failure.
