• Integumentary SKIN:
System • Dry and wrinkled
• Turgor decreased
• Veins more prominent
• Presence of lentigines in the
extremities

HAIR:
• Thin, gray, fine and straight
• Distribution is not even

NAIL:
• Nail edges are smooth ,
rounded and clean
• Nail plate is translucent and
pale
• Color return is instant and
• at least within a few seconds

FACE:
• Nose and mouth prominent
• Mouth sunken because of
lost teeth

EYES:
• Presbyopia
• Sunken
• Dry due to decreased tear
production
• 20/40 vision

Health History:
➢ Had history of arthritis or
joint pains
➢ Experiencing back pain
➢ Absence of bone pain
➢ No bone deformity
➢ Absence of swelling
and swelling in the joints.
➢ Presence of back
pain without
• MUSKULOSKELETAL tenderness
SYSTEM

Upper Extremity:
• No redness, muscular
atrophy, deformity or
swelling is present
• Size and contour of the
shoulder are equal
• The spine is slightly curved.
• Absence of lesions or
swelling in the wrist and
hand
• Can extend and flex the
wrist with limitation

Lower Extremity:
• Can extend and flex the
knees with limitation
• Pain upon bending the spine
