We are pleased to inform you that we have scheduled your personal appearance to
this Office for the Career Service Examination - Computer-Assisted Test (CSE-CAT)
for the First Quarter of 2011. Please proceed to Examination, Recruitment and
Placement Office (ERPO) of the Civil Service Commission - Central Office (IBP Road,
Batasan Hills, Quezon City [View Map]) on the date and time indicated below for the
filing and processing of your CSE-CAT Application.

Time : 2:00PM - 3:00PM

Please print a copy of this letter and bring it with you on the day of your personal
appearance, together with your duly accomplished Application Form (CS Form No.
100, Revised 2008), original and photocopy of any valid I.D. card with name, picture
and signature of the applicant, and name and signature of the issuing officer (e.g.
Driver's License, BIR ID, GSIS/SSS ID, Postal ID, Voter's ID, current Office/School
ID, or valid Passport), four (4) pieces 1.5" x 2" ID picture with full nametag (taken
within the last three months prior to filing of application) NOTE: The following
pictures are NOT accepted:
      – Scanned, computer-enhanced, photocopied, and cut-out pictures;
       – Pictures showing subject applicant not in close-up shot (lower than shoulder
level);
       – Pictures with incomplete and/or computer-generated name tag; and
       – Pictures without name tag.
and payment of examination fee of Php 400.00.

ATTENTION:

1. Please note that the date and time indicated above DO NOT pertain to an
actual examination schedule of the CSE-CAT but on the filing and processing
of your application form. This schedule of personal appearance is not
transferable and valid only on the date and time specified above.

2. The reservation made using the Online CSE-CAT Application Reservation

System is non-transferrable.
