the payment in an envelope and put your name on it.
STEP 3: Submit this form along with the payment to one of the organizers.
STEP 4: Go find a date!

NOTE: This $40 deposit is non-refundable if you cancel your reservation, or fail to
submit your second payment by APRIL 29, 2011. No exceptions.

Payment Policy
This contract is deemed void unless accompanied by a $40, non-refundable deposit. This $40 deposit will be
acknowledged towards the final payment of a completed reservation ($145), upon which a second payment of $105 will
be due before April 29th, 2011. This deposit will be forfeited should the undersigned cancel his/her reservation or by
failure to complete the payment by April 29th, 2011.
Terms & Conditions
This contract is a revocable license subject to termination at any time at the discretion of the organizer(s) of PW Prom
2011. This contract entitles the individual ONE reserved seat at PW Prom 2011. The organizer(s) withhold the right to
process reservations according to availability without the explicit or implicit consent of the undersigned, provided no
preferences are indicated by April 29th, 2011. No reservations will be held past April 29, 2011. All deposits and
reservations will be forfeited by the undersigned past this date.
