Student form for non-Springboro students

In order to attend Springboro’s High School dance, you must fill out and
return this form to Springboro when you purchase a ticket during the week
of April 25th, 2011. Tickets will not be sold until this form is completed and
returned at the time of ticket sales. If permission is granted, the person may
purchase a ticket.

This needs to be completed by the person who is NOT a Springboro student.
