I,______________________, plan to participate in the various activities offered by Filipino

Americans Coming Together on Saturday, May 7th, 2011, at the event·s grounds at his/her
own risk. I do hereby release the Filipino Americans Coming Together, sponsors and staff of
the F.A.C.T. Field Day from any liability due to accident or injuries incurred during the day·s
activities. I will participate in Field Day in an appropriate manner and be supportive of my
team as well as other teams. I will be a good sport. I am aware that F.A.C.T, Field Day
participants, volunteers, and the park are not responsible for any missing/damaged personal
belongings, or any actions/injuries/harm done due to these actions. Every reasonable effort
will be made to insure the safety of all participants and myself.
