Wednesday April 27th 6:00 pm (Lp Dance Room)

* Mandatory Try-Out Clinic

Wednesday & Thursday May 4-5th 3:15-6:30 pm (Lp Dance Room)

* PDC Audition:
Friday May 6th 3:15pm - Check in (Lp Dance Room)
3:30-6:30 pm - Auditon

Please return the following completed forms signed and stapled to the audition.
These forms must be turned in on time to be eligible to audition.

#3 Parent Permission Form blue
#4 Current Transcript (signed by your counselor)
#5 4 (Four) Teacher Recommendations *

* Every person auditioning MUST have 4 “Teacher Recommendation Form” and turned in (by the
teacher) to Mrs. Di Lello’s box. These are confidential! If you have taken dance in the school, one must
be completed by the dance teacher, other wise do all by your current teachers.
