Essay Questions for InTACT Student Facilitator Applicants:


On a separate sheet, answer each question as thoroughly and concisely as you
can within 10 to 15 sentences. Use font type ARIAL font size 10, single
space.

1. Why do you want to become an InTACT Student Facilitator?


2. What, for you, is the purpose of the InTACT Program?
3. What personal characteristics do you think/feel are assets in being an InTACT
Student Facilitator? Please expound on your answer.
4. On a scale of 1 – 10, 10 being the highest, how willing are you to be part of the
InTACT program as a Student Facilitator.

Submission Instructions:
1.Attach a colored 1x1 ID picture on the upper-left side of this Information
Sheet.
2. Fill out this Information Sheet completely and accurately.
3. Answer the Essay Questions on a separate sheet of paper following the
above mentioned format.
4. Provide a copy of your Transcript of Records (1st yr. - Present). A Print-
out of your grades from AISIS will suffice.
5. Accomplish the Recommendation form. Specific instructions are to be found
in the form document.
6. Staple all these documents together and submit them at the ADSA Office
(Window 1 - Formation); soft copies are also to be e-mailed to
intactrec1112@gmail.com on or before 5pm of April 25, 2011 (Monday).
