Managerial Assessment Information Required.

Dear member of the Mbombela Management Team.

You will now recall that Talentline has almost completed the milestones for the Organisational
Health Check as well as for the Organisational Structure. The results of the Workshop, held on these
topics, will be shared with you shortly.

Our next milestone will be to perform a 360 degree leadership assessment on the top 24 officials at
Mbombela. We are mindful of your busy schedules and would thus like to share the following
information with you.

 The purpose of the assessment is for us to obtain inputs from you, as well as from staff with
whom you interact closely, regarding your leadership capabilities. This MUST NOT be seen as a
performance management tool, but rather one that will assist you in identifying possible
development areas. This information will then be used to guide the process of coaching and
mentoring thereafter.

         The completion of the assessment will be done online in questionnaire fashion.

         You will have to identify the following persons and fill it in on the attached document.

o   Details of your supervisor

o   Details of at least 2 peers with whom you have regular work contact

o   At least 3 Subordinates who report directly to you.
