This questionnaire is intended for evolutional purpose only. It will be
followed by a physical visit to the institution by the undersigned officer.
Please facilitate the officer by duly completing the questionnaire in its
totality and very honestly.

1. CONDUCT OF THE PHASED PRACICALS

a. Out of the six (6) phased practical sent to your institution, how many were your
students able to do? (Indicate course and the number of phased practical per

b. When were the materials for practical availed to the student?_________________
c. What challenges did you encounter during the phased practicals?______________
d. What type of tools and equipment did you use for the practicals? (Indicate course
