I, VALERIAN BUCHADI RWEBANDIZA, of P.O. Box 68715, DAR ES SALAAM, HEREBY applies for the change of signature in the land register. I solemnly and sincerely declare as follows;1. That I am the owner of the above mentioned Certificate of Title which is registered in the name VALERIAN BUCHADI RWEBANDIZA, of P.O. Box 68715, DAR ES SALAAM. Attached herewith marked VBR-1 is a copy of the said Certificate of Title.

2. That I had signed on the Certificate of Title in 2009 and since then I had not been commonly using the said signature and hence I cannot reproduce it.

3. That also further to your letter dated 16th December 2010 with Ref. No. LR/T/84914/14 and due to the above reason I hereby apply to change the former signature and replace it with a new one. 4. That all what is stated herein above is true to the best of my own knowledge. That I make this solemn declaration conscientiously believing the same to be true and in accordance with the provisions of the Oaths (Judicial Proceedings) and Statutory Declaration Act, 1966.
