Title: Effects of mistakes on Quality Have you ever made a mistake? If you answered no, then that is a mistake, because we are all human and nobody is perfect, mistakes will be made, humans are fallible. Definition of mistake: A mistake is the result of an action a person performs where the outcome is an error (something is not right).

Mistakes have an effect on Quality of work where the consequence of the mistake has the potential to render an item unacceptable to minimum quality requirements or the quality of the item is indeterminate. The reason for a mistake could be attributable to but not limited to: Poorly written procedure Poorly written Work Order Poor communication either written or verbal Poor or misguided judgment Ignorance Inattention to detail Poor work attitude poor work ethic Misunderstanding of requirements Less than adequate direction from Management or Supervision Less than adequate equipment or tools Incorrect use of tools Less than adequate training

When mistakes are realized or discovered personnel should stop work, and notify their supervision to not only identify the mistake but to obtain the necessary action to move forward. Generally, mistakes are unintentional and no one should feel wrongly about making a mistake or feel they will be disciplined for making a mistake. We can all learn from our mistakes and gain additional knowledge and experience which is one reason why we have a Lessons Learned Program and a Corrective Action Program. These programs when utilized correctly can enhance our ability for continuous improvement. In closing: You do not want to cover up a mistake; this only leads to a larger mistake or larger problem.
