The payment Plans I/We choose to make our payment is pursuant to the following:
______ One-time full tuition paymentmust be paid by July 15, 2011
______ Monthly payments--July 2011 through April 2012 using the FACTS Program.
FACTS will deduct their $38 administrative fee from the account you specify.

Non-Sufficient Funds Policy (NSF) Tuition payments received with Insufficient Funds will be
charged a $30 handling fee plus the bank fee.

I /We agree to the terms of this Family Investment Contract. I/We understand that this Family Investment
Contract is binding and that I/we are responsible for its contents. Payment of the registration fee(s) and a
signature on the Family Investment Contract will complete the registration process. Please make checks
payable to Guardian Angels School.
