RECOMMENDATIONS:

1) To conduct a research study regarding the status oI medication errors in the institution.
2) To conduct a campaign against medication errors.
3) To develop a continuous education program Ior nurses, including other health care workers
involved with drug administration regarding knowledge, skills and attitude in drug
administration up to date.
4) To develop strategies on how to prevent medication errors this may be the Iollowing:
* (based on the Iindings and conclusions oI the presented research study)
A) To encourage the creation oI a "culture of safety" in which medical errors are
discussed openly and addressed thoroughly. When an organization values saIety this
commitment is evident throughout the organization Irom top management to the bedside.
A culture oI saIety includes:
O Acknowledgment oI the high risk, error-prone nature oI an organization's activities.
O reation oI a blame-Iree environment where individuals are able to report errors or close
calls without punishment.
O pectation oI collaboration across ranks to seek solutions to vulnerabilities.
O Willingness on the part oI the organization to direct resources to address saIety concerns.

B) Improve the eIIectiveness oI communication among caregivers.
O or verbal or telephone orders or Ior telephonic reporting, veriIy the complete order by
having the person receiving the order "read-back" the complete order or test result.
O or physicians to limit the use oI abbreviations especially those that may seem to have
similar meanings. (e.g. IU (International Unit) ,mistaken Ior IV (intravenous), or the
number 10 (ten ) Write "International Unit"
O urses shall veriIy physician`s orders that are not legible enough Ior them.

) Give importance on accurately and completely reconcile medications across the
continuum oI care.
O Implement a process Ior comparing the patient's current medications with those ordered
Ior the patient while under care oI the organization.
O A complete list oI the patient's medications is communicated to the net provider oI
service when a patient is reIerred to or transIerred to another setting, service, practitioner
or level oI care within or outside the organization. The complete list oI medications is
also provided to the patient on discharge Irom the Iacility.
