Mendoza, Rizielle Anne S. 2NUR7 Patient is S. G.

, female, 34 years old, married with 2 kids and a resident of 3360 Policarpio St., Gen. T. De Leon, Valenzuela City. She is a nursing graduate of Fatima University. Patient currently has no job and stays at home but worked as a sales supervisor in a pharmaceutical company for 12 years. Patient is a Roman Catholic. Patient had her first menstrual period when she was 14 years old. Patient said that she has regular interval of menstrual period every 28 days that lasts for 4-5 days. Patient only experiences dysmenorrhea every first day of her period. To relieve the pain, she lies down and apply hot compress on her lower abdominal quadrant. Patient said that there s no other accompanying symptoms that comes with her dysmenorrhea. Patient was 34 AOG (G2P1) when admitted last September 16, 2011. Her LMP was last January 13-16, 2011. Her EDB should be between 0ctober 20-22, 2011. She had her first baby, which is a boy, via CS section last 2008. She had a smooth pregnancy back then. Patient was admitted last September 16, 2011 due to complains of uterine contractions every 4 hours with duration of 2 minutes. Further tests were conducted and it revealed that she has placenta percreta. Patient gave birth to a live baby girl delivered via classical CS last September 18, 2011. Patient also underwent urethral stent insertion and hysterectomy. Patient has a family history of cancer, diabetes mellitus and asthma on mother s side. Patient haven t had any major operations other than CS. She had a history of preeclampsia during her first pregnancy and was given anti-hypertensive only for 1 month. Patient is allergic to seafood and takes vitamin C at home. Patient enjoys taking care of her children and husband, cooking and playing computer games. She doesn t have any restrictions when it comes to food and doesn t go on a diet. Patient also doesn t exercise. She usually sleeps 8 hours a day and feels well-rested. Patient is a non-smoker, non-alcoholic and denies illicit drug use.

Patient described relationship with husband as contented but sometimes challenging. Patient says that their strong faith in God makes them strong and they never go to sleep without resolving their issues. Patient describe home environment as stable. Their house is made of concrete materials and says that her family gets along with the neighbors. Patient only expressed concern regarding flood in their place especially when it rains hard.
