A. Personal Data and History (Demographic Data) Mrs. M is a 64 years old female, married living at 1442 Atis St. Garcia Subd. San Antonio, Binan, Laguna. She was born on Aug. 11, 1947 in Pasay City. She is married for 43 years now and has 3 children. According to mrs. M, educationis important thats why she decided to look for more affordable career. While studying she decided to work to be able to support her education. With her perseverance and determination, she was able to finished Bachelor of Science in education. And has been teaching for almost 47 years. Mrs. M doesnt smoke nor drink alcoholic beverages. Mrs. M said that she is fond of eating oily and salty food and drink a lot of soft drinks. Before going to work, sometimes she help her auntie to sell fruits and tapper wear. At present, she is just at home with her husband and mother. Mrs. M and her husband dont have a work and they are getting their finances to their children, who have their own families. Mrs. M was admitted in Jonelta last Oct. 2011. She was admitted due to chest pain. B. Family History (+) HPN (+) DM C. History of Past Illness Mrs. M was known of being hypertensive for 19 years now. She was diagnosed of hypertension and kidney failure year 1992. She was hospitalized and discovers that her right kidney was already dead and her left kidney is weak and has nephrolitiasis. On Oct. 2011, she was hospitalized again in Jonelta. Mrs. M experienced chest pain. D. History of Present Illness Few hours prior to confinement, Mrs. M experienced difficulty of breathing, no associated symptoms noted. Patient had self-medicated with Catapres and Isordil which afforded temporary relief. No consult done due to persistence symptoms. Mrs. M sought consult to Jonelta.
