Pramod Yadav , yrs , male patient, resident of Bihar came with c/o blurring of vision since 4 months

 On Exmn concious , oriented Higher Mental Functions/speech Normal VA Right Left Fundus showed

Showed lesion of size clivus which was Hypointense on T1 Hyperintense on T2 enhancing on contrast s/o clival chordoma arising from the

 Informed consent taken from the relatives and explained regarding the need for emergency surgery and prognosis.

 Surgery Transnasal Transsphenoidal approach and decompression done on

IntraOP

POD 1
Post OP period patient had bleeding from the operated site and was manged conservatively

Post OP CT scan s/o -

POD 2
The patient was kept in ICU for observation. Nasal pack was insitu with lumbar drain in place The patient was disoriented with GCS E4V4M6

POD 3
The patient sensorium improved and was shifted to ward Serum electrolytes were monitored

POD 4 to POD 8

POD 9
The patient was unresponsive suddenly with GCS E2V1M5 , vitals stable and was shifted to ICU again Serum electrolytes repeat showed hyponatremia ( Sodium ) Sodium correction was started

POD 10
The patient was drowsy , hypoxic So intubated and started on sedation

 Repeat sodium showed

 The patient further detieorated with GCS and condition explained to the relatives

 The patient was finally declared dead on
