A copy of the transcript and audio recording of the October 14th Public Hearing regarding Item No. 11. On October 14 and November 4, 2010, MOCS held two public hearings at 10 a.m. at Spector Hall, 22 Reade Street, Main Floor (respectively, "October 14th Public Hearing” and "November 4th Public Hearing"), regarding, among other things, the Proposed Contract. The October 14th Public Hearing on the Proposed Contract is referenced as "No. 11, Department of Homeless Services" on the Calendar of Public Hearings On Contract Awards, dated October 14, 2010 ("Item No. 11"), the relevant portions of which are attached hereto as Exhibit A. The November 4th Public Hearing on the Proposed Contract is referenced as "No. 6, Department of Homeless Services" on the Calendar of Public Hearings On Contract Awards, dated November 4, 2010 ("Item No. 6"), the relevant portions of which are attached hereto as Exhibit B. All documents, including, but not limited to, written statements, correspondence, memoranda, letters, e-mails, facsimiles or electronic documents, including all attachments or documents related thereto, submitted to MOCS by any person or entity, including, but not limited to (i) BRC; (ii) BRC Executive Director Muzzy Rosenblatt; (iii) 127 West 25th LLC, the owner the property where the Proposed Contract services will be provided; (iv) Daniel Shavolian, the purported sole member of 127 West 25th LLC; (v) any agent, representative, contractor or attomey of BRC, 127 West 25th LLC or Daniel Shavolian; (vi) DHS; or (vii) any City or State agency, at any time, whether before, at, after, or otherwise, arising out of, in relation to or regarding the October 14th Public Hearing, the November 4th Public Hearing, Item No. 11, Item No. 6 or the Proposed Contract. 4. All communications between or among MOCS and BRC, Muzzy Rosenblatt, 127 West 25th LLC, Daniel Shavolian, DHS, or any agent, officer, director, employee, representative, contractor or attorney of BRC, 127 West 25th LLC or Daniel Shavolian, or any City or State agency, arising out of, in relation to or regarding the October 14th Public Hearing, the November 4th Public Hearing, Item No. 11, Item No. 6 or the Proposed Contract. 5. All communications in MOCS' possession between or among BRC, Muzzy Rosenblatt, 127 West 25th LLC, Daniel Shavolian, DHS, or any agent, officer, director, employee, representative, contractor or attorney of BRC, 127 West 25th LLC or Daniel Shavolian, or any City or State agency, arising out of, in relation to or regarding the October 14th Public Hearing, the November 4th Public Hearing, Item No. 11, Item No. 6 or the Proposed Contract.
