PARENT/GUARDIAN PERMISSION FOR STUDENT RIDING IN PRIVATE VEHICLE FOR SCHOOL SPONSORED ACTIVITY

Dear Parent: Certain circumstances merit the use of private vehicles for the following school activity: Date 3/3/12 Destination USC Purpose JETS TEAMS Competition Driver: Teacher Other Parent Student (Must have valid California Drivers License; provisional license not allowed) Time: Departure 7 am Return 3 pm

Our district's policy requires that the trip must be optional and students cannot attend without the consent of the parent/guardian. Although care and supervision will be provided, your child will not be covered by the District's automobile liability insurance policy. We do require and verify, however, that each driver carry insurance of not less than $100,000 per person and $300,000 per accident bodily injury, property damage liability limits of $50,000, and $2,000 medical payments. The driver will provide the Claremont Unified School District with authorization to check his/her driving record through the California Department of Motor Vehicles Driver Record Program. The drivers license must be valid, and driving records must be acceptable to the District. The private vehicle must be registered in California and be in proper mechanical condition. The vehicle cannot be overloaded and must not carry passengers that exceed the number of seat belts in the vehicle. Please sign below indicating that you consent to have your child participate in this school activity under the conditions described and that you waive all claims and hold harmless the individual sponsor(s), the Claremont Unified School District, and the State of California for any injury, accident, illness or death or any loss or damage to personal property occurring to your son/daughter named below during or by reason of the trip/excursion described above.
