DETAILS : Muhammad had switched on the switch of electrical saw to cut the steel rod that need to be used in Production Line. Without wearing any safety equipment and less of experience to use that machine, he start to use it. When he start to use the electrical saw, his finger was putted on the steel rod while the electrical saw start working. The electrical saw cut his left finger and he start to yell. Meanwhile, The Head Maintenance, Hisham heard his yelling and running toward him. Hisham take him to company clinic to take earlier treatment while waiting for ambulance. Ambulanced reached to the company at 3.50 p.m. He was taken to the nearest hospital for further treatment. C. CAUSES After an initial investigation, I discover that Muhammad does not follow Standard Operation Procedure (SOP) correctly. The procedure request user need to used and wear a safety ewuipment such as glasses and glove and stay a little bit far from the machine to ensure theres no accident happen. A further investigation revealed that Muhammad is a new workers and has no experienced to handle that machine D. RECOMMENDATIONS To prevent other similar occurrences, I suggest the following : y Always read SOP before start working or using the machines. y Wear a safety equipment when you using a machine especially those machine that take a high risk while using it.

As follow-up, these action have been taken : ACTION NO. 1 DESCRIPTION SOP has put nearer and can be seen ASSIGNED TO Hisyam DATE 10 March 2009
