Consumer Support Grant (CSG)


What?
The CSG covers several home care services including health aides, Personal Care Assistants, and private duty nursing. The CSG program is a state funded alternative to Medicaid reimbursed home care, which uses both state and federal funds. The CSG program allows individuals to convert the state funds for specific home care services into a cash grant. These cash grants allow individuals to have greater flexibility and more choices for many different home and community based services. This option allows individuals to pay parents, family members, and neighbors for services as well as traditional providers. Services that can be paid for through this grant:

Who?
To be eligible, an individual must: Either receive or be eligible for Medical Assistance (MA) Be eligible to receive home care services through MA Display deficits in every day functioning Require supports to successfully live in the community Live in a natural home setting Not participate in Home and Community Based Services/Waivers These requirements are determined based on home care assessments conducted by public health and county social services.

Companionship services Home/vehicle modifications Nutrition services Respite Transportation And many more

How?
An individual can receive the Consumer Support Grant in the form of cash, vouchers for services, or direct payment to providers. Applications for the CSG program can be submitted to your county Department of Human Services though a disability social worker. The CSG is not currently available in all counties in Minnesota. If you live in one of these counties you can ask your county Department of Human Services to consider adding the CSG.
