Cooperation Rubric

Exemplary
Student consistently meets classroom expectations, always promotes a cooperative group environment and has NO discipline actions on his/her cooperation file.

Satisfactory
Student generally meets classroom expectations, works well within his or her group and has between 1 and 4 discipline actions recorded on his/her cooperation file.

Unsatisfactory
Student rarely meets classroom expectations, consistently disrupts the classroom environment and has 5 or more discipline actions recorded on his/her cooperation file.

Codes for Disciplinary Action

(1) Not following primary class procedures (i.e. seated by tardy bell w/ materials) (2) Interfering w/ learning environment (i.e. being off-task, talking/out) (3) Disrespecting others w/ inappropriate language or behavior (4) Being defiant! (5) Incomplete Behavior Assignment (i.e. w/out parent signature, too sloppy) (6) No effort to improve (i.e. failure to remain for counseling, 3 consec. days of #5)
