Dear Mr. Switzer, On behalf of CQ* I am pleased to offer this proposal to HCC in response to your Request for Proposal titled Business Ethics Training. The proposal includes a project plan and budget for the development of two course module portals. Our proposal meets the requirements set forth in your Request for Proposal and is effective from April 8, 2011 to May 8, 2011. After personally reviewing the proposal with management, key staff, as well as the selected project team, I am confident that the enclosed solution is both sensible and empowering. As you will see, our solution clearly defines how all, critical factors are addressed. Our experienced professional staff, proven instructional design process, and state-of-the-industry learning design systems make CQ* an excellent choice for completing the project on time and within budget. Executive Summary
Challenge: CQ*s challenge to HCC is to propose a means of developing an interactive multimedia training program called Business Ethics Training. This program will be delivered via web module through HCCs intranet system. It will serve as a self-paced certification course and content refresher. Solution: Instructional CQ has created a multimedia training solution to include a self-paced certification tutorial with interactive questions, scenario and response systems and is webbased. Technical Due to the use of video in the Business Ethics Training products, the best technical solution is to deliver content via an Internet portal and package the material for the Internet Explorer 9.0. This approach will allow the quickest viewing of the largest amount of video and multimedia. Deliverables: CQ* can begin work by May 18, 2011. A beta version of the program will be available for evaluation by June 30, 2011, and the final product will be available by July 13, 2011. In addition to the final web module, CQ* will also provide hard copies of all manuals and documents. Price: The proposed cost for CQ* to meet the challenges of HCC will be $80,745. 1 Analysis Phase
CQ* will conduct an analysis phase to solidify learning objectives for the audience of senior and mid-level managers and to evaluate the extent of Health Care Cooperatives capacity to support the best e-learning tools to fit these objectives. This phase will include an individual needs analysis, content analysis, and analysis of technology. A brief summary of each of these phases follows: Individual Needs Analysis: To best serve the audience of senior and mid-level managers, CQ* will assess the learning styles and preferred modes of information delivery to create a learning tool that best suits the needs of this group. Through an electronic questionnaire and, as needed, individual interviews, CQ* will gather information directly. Also CQ* would ask to review job descriptions, and HCCs organizational map of employees to specifically assess what each manager does, who they are responsible for, as well as who they report to in order to create an easily relatable and realistic learning tool. Content Analysis: This phase of the assessment would require CQ* along with the SMEs that HCC provides, to monitor and review any documents, employee manuals and information used on the job. A review of the information sources that managers currently use to make decisions is especially necessary to create an applicable training course for the appropriate use of company assets module. Technology Analysis: CQ* and HCCs Information Technology department will assess the technological capabilities of the computer software and hardware available to ensure that the educational solutions we propose can fit the constraints of the environment. Below is a graphic depiction of these analyses, which we hope to conduct concurrently as a formative process, revising during the entire process with Health Care Cooperatives continual feedback.

Solution
The Course Modules: Here is what CQ* envisions for an effective e-Learning solution as HCC embarks on a new path of training for its senior and middle level managers aimed at the topics of employee personal conduct and appropriate use of company assets.

2.1 Employee Personal Conduct Course:

CQ* recommends a self-paced tutorial as an asynchronous learning experience focusing on expectations and policies for employee personal conduct within HCC. Since HCC has office locations throughout the United States, the need for learners to assimilate the information first is important so an asynchronous selfpaced tutorial will accommodate managers who have varying demands on their schedule, working in different time zones. Because HCC is seeking to provide alternative options for training which simulate the previous on-site presentations and collaborative opportunities, the course module will include: Case studies (these are scenarios presented to the learner based on specific HCC policies for employee personal conduct)

Quick knowledge checks with immediate feedback (These quick checks provide the learner with the opportunity to check his/her understanding of the concepts presented while receiving immediate feedback for those results) Interactions will allow the learner to process through the information presented in the course. These interactions provide the opportunity to present the cause and effect relationship of employees conduct within the company. Scavenger Hunt (send employees through HCCs employment manual to find particular policies and guidelines.) A follow-up collaborative opportunity will include an asynchronous discussion forum. An asynchronous program allows the learner to interact with learners in a flexible time frame. Discussions are an integral part of understanding how to promote a positive work environment which will also support HCCs five foundational principles. This will provide an opportunity for these managers to discuss, share and work with one another no matter where they are in the country. This can be used to promote a positive work environment through a sharing of experiences both positive and negative. This forum can also be used for advice or for bragging about employees etc. A deliverable will be provided to the senior and mid-level managers so that they can bring the information they learned to their employees. This take-away can be used to share information with the employees and to start a dialogue with the employees about personal conduct. For tracking purposes a certificate will be issued at the end of the course, which can then be sent to the HR department to demonstrate course completion.

2.2 The Appropriate Use of Company Assets Course:

CQ* recommends a self-paced tutorial option similar to the employee conduct course. This course adapts to the pressing demands of the managerial staff and encourages the need for collaboration among peers, focusing on expectations and policies for using HCC assets appropriately. Course content will include a presentation of the learning objectives, current legislation and HCCs policies on the use of company assets. For tracking purposes a certificate could be issued at the end of the course, which can then be sent to the HR department to demonstrate course completion Case studies (these are scenarios presented to the learner based on specific HCC policies for employee personal conduct) During the presentation, audio/video software will be used to present practical scenarios that senior and mid-level managers may face in their day-to-day work when using HCCs company assets. A mastery test will be developed as part of the completion for the course. For tracking purposes a certificate could be issued at the end of the course, which can then be sent to the HR department to demonstrate course completion A deliverable will be provided for managers to share with employees.

CQ* will provide a DVD video and scripts to role-play scenarios. Stopping points throughout the script will enable discussion breaks through leading/guiding questions. These key questions are an integral part of developing and understanding how to use HCCs assets appropriately. The senior manager in the office will be the facilitator of the group discussion activity.

Deployment and Support


Throughout the process of the course development, implementation, and support, CQ* will maintain constant contact with HCC to ensure clear communication. Once CQ* has been awarded the project we will set up a kick-off meeting with HCC in order to develop a clear plan of progress with all necessary parties. During development, CQ* will hold ongoing meetings with the subject matter experts to be sure proper course content is being included in each course. The timeline may be affected by response time with the SME but we factored in that time to the best of our abilities. Once a course is complete CQ* will meet with HCC to test the product to ensure accurate course content and acceptability of product. CQ* will then work with HCCs IT department to place the course on HCCs intranet system. CQ* will provide IT support to ensure quality and workability of the product throughout its use within HCC. The IT support cost has been included for the first month after implementation. After the first month, CQ* offers IT support to all of its clients on an as needed basis. Below is a chart that explains the costs for 6 months or 25 hours of support, if a client opts to use CQ*s IT services. Optional Technology Support 6 months or 25 hours of IT support Cost $2500.00

3.1 Timeline CQ* has developed a timeline that provides an approximate deployment date of July 13, 2011 for each course. We will develop each course concurrently so that HCC is able to utilize these courses as quickly as possible. Below is an outline of this timeline:

3.2 Risk Analysis/Mitigation In the event that CQ* experiences personnel changes, we have a large and qualified staff able to mitigate these unexpected scenarios and keep your project running and on time within your requested timeline. Our thorough project documentation process and commitment to collaborative teamwork help create an atmosphere where we can say with confidence that any team member can help execute: Any project plan At any time On time. In rare circumstances, occurrences may arise which may affect the cost or timeline for the completion of this project. CQ* will notify HCC if we believe there may be any sort of delay or change in the projected cost. We will work closely with HCC to resolve the situation to the satisfaction of both companies. 3.3 Corporate Capabilities Founded by Wayne Kroeplin in 1995, CQ* has grown from a small, four employee company in Milwaukee, WI, to a company of 23 highly skilled professionals. It was during the early 90s, as Vice President of Human Resources at Wisconsin Public Service Health Corporation, Kroeplin saw a decline in the size of his training department due to company reorganization. He recognized the opportunity to become an external training provider to the health care industry and other companies who were realizing that many traditional training techniques were not sufficient enough to effectively move knowledge from the workplace to the worker. As companies are spread farther apart, training becomes more expensive. However, with modern technology such as the Internet/ Intranet cost effective and successful training can be achieved. CQ* is a leader in the e-Learning field. We work hard to meet the needs of our clients providing a range of training opportunities from in-person training, to blended learning experiences, to a total e-Learning experience. CQ* will work tirelessly with a client to understand their needs and develop trainings specific to their organization. So now what can CQ* do for you? Company History 1995 - Wayne Kroeplin started his company by providing instructor-led training for the insurance industry. 1997 - CQ* quickly expanded its services to include performance-based training solutions for financial institutions in the areas of new employee orientations and employee relations.

1998 - Winner, Brandon Hall Research award for best use of blended learning

2001 - CQ* is recognized by the American Society for Training and Development (ASTD) for outstanding E-Learning Development in the Health Care Sector 2003 - Our Learning Management System was developed to create efficient and effective learner-focused instruction. 2004 - Received CPLP Certification (Certified Professional in Learning and Performance credential) 2008 - CQ* continues to lead the organization in developing leading and innovative learning solutions. Its Research & Development team has continued investigations in the integration of instructional best-practices that strive to meet the growing needs of clients world-wide. The core project team has been working together since 2002. Each member of this core team will work throughout the entire length of the project. Additional workforce personnel, such as additional multimedia designers, database administrators, and a quality assurance specialist, will join the core team as is necessary. Key Project Members Wayne Kroeplin, Project manager: Wayne has worked in general corporate for 9 years and the instructional technology field for over 10 years. He is responsible for budget, resources, and scheduling. Wayne also manages workload, processes, and communication for the team. Related Experience: All core team members have worked together since 2002 and have won numerous awards in the areas of e-learning, blended-learning and mobile learning. One other factor to take into account is time. With e-Learning, managers are able to participate in each course as his/her schedule allows.
