Dear Dr. Gisbert,

Unfortunately, I am unable to accompany my dad to this appointment so I will give you a brief history of his health. About 5 years ago he suffered a mild stroke and he is a diabetic. He was at that time given blood thinners and an anti-depressant (my mother had just died) by Dr. Vince Macalintal and after a year or so he stopped the medication as he felt it was no longer required. Also, while he was on blood thinners he always had hematomas under the skin. His sugar is under control and his insulin has been cut down 3 times by Dra. Gia Wassmer. Yesterday I noticed that his foot was red, swollen and he complained of some pain so I suggested that the caregiver bring him to you to check this out. She carries with her a list of all his medication and his last blood works result. He is also hard of hearing so I asked the caregiver to take down all your recommendations and instructions. He is not walking for exercise anymore because he gets dizzy spells which we have run tests for time and again but it seems that it comes with his hearing problem and possibly the diabetes. He is mobile at home and still goes out but not as much as we would want him to. He also has the stockings which he refuses to wear.
