R Street, Muslimpur ,Vaniyambadi, Vellore District, Tamil Nadu State. Do hereby solemnly affirm and state as follows: 1. My original name is O.Imraz and my fathers original name is O.Amjad Basha. I have enclosed a xerox copies of T.C and 10th Marksheet in which original name was mentioned. 2. At the time of creating birth certificate IMC.A.No.1208199, Comissioner Vaniyambadi Municipality wrote my name as Imraz instead of O.Imraz and my father name Amjad Basha instead of O.Amjad Basha which is not a correct one. 3. The absence of initial O in my name and my fathers name in the birth certificate led a problem while applying for a passport.

4. So i need affidavit to show to the passport authority to accept my name as O.Imraz and my fathers name as O.Amjad Basha. 5. I take oath and solemnly declare that the particulars furnished by me above are correct and that have not concealed on mis represented any facts.
