AFFIDAVIT OF LOSS

I, CAMILLA ANN L. CASTAEDA, Filipino, of legal age, and a resident of Block 3, Lot 5, Ciudad Del Carmen Subdivision, C. Raymundo Avenue, Barangay Rosario, Pasig City, after having been duly sworn in accordance with law, hereby depose and say: 1. That I am a Safety Deposit Box holder of EXPORT AND INDUSTRY BANK, San Miguel as evidenced by the hereto attached copy of the receipt of payment of rental fee dated 2 January 2012; 2. That sometime in the month of January 2012, I discovered that my key to the said Safety Deposit Box was no longer on my table where I used to place it; 3. Despite diligent search and efforts to locate said document, I could not find it; 4. That I am executing this affidavit to attest to the truth of the foregoing facts and to request for a replacement of my key to the said Safety Deposit Box;
