EHR Meaningful Use Configuration Checklist

Stage 1 CORE Objectives: 15 of 15 Required


Core Objective 1 - Orders & Eprescribing >30% w/at least one medication in medication list using CPOE Core Objective 2 - Drug-Drug / Drug Allergy Interactions Must have functionality enabled for the entire EHR reporting period Core Objective 3 - Orders & Eprescribing >40% of all permissible prescriptions using certified EHR Core Objective 4 - Demographics For >50% of patients, record gender, D.O.B., race (Am Ind or AK Nat, Asian, Blk or Afr Amer, Nt Hl or OTPa Is, White, Declined) ethnicity (His or Latino, Not His or Latino, Declined) & preferred language Core Objective 5 - Problem List > 80% of patients must have at least one diagnostic entry or indication of no problems Core Objective 6 - Medication List > 80% of patients must have at least one entry or indication of no prescribed medication recorded as structured data Core Objective 7 - Medication Allergy List > 80% of unique patients must have at least one entry or indication of no prescribed medication recorded Core Objective 8 - Vitals For > 50% of patients age 2 & over, record changes in vital signs including: Height, weight & blood pressure. Calculate & display BMI. Plot & display growth charts for children age 2-20 including BMI Core Objective 9 - Smoking Status Record smoking status for > 50% of patients age 13 & older. Tie smoking habits Meaningful Use to clinical data for reporting. Core Objective 10 - Clinical Decision Support Rule Implement one clinical decision support rule relevant to specialty or high clinical priority with ability to track compliance. One option is to enable & use health maintenance protocols for RX's. Must attest that this function is in use. Core Objective 11 - Clinical Quality Measures Report on 3 core measures: Hypertension (blood pressure); Preventative care; Screening Pair: tobacco use & cessation intervention; Adult weight screening with follow-up. Core Objective 12 - Electronic Copy Provide patients with electronic copy of their health information including diagnostic test results, problem list, medication list & medication allergies within 3 days of patient request Core Objective 13 - Clinical Summary >50% of patients must be provided with a clinical summary within 3 days of their visit containing an updated medication list, lab & other diagnostic test orders, procedures & other instructions based on clinical discussions during patient visit Core Objective 14 - Electronically Exchange Key Clinical Information Perform at least one test of certified EHR technology's capacity to electronically exchange key clinical information through importing or exporting patient data via CCR or CCD files Core Objective 15 - Security Administrator's Application Guide Conduct or review security risk analysis and implement security updates as necessary and correct identified security deficiencies as part of its risk management process

Menu Objective 1 - Implement Drug Formulary Checks Must have functionality enabled and have access to at least one internal or external drug formulary for entire EHR reporting period Menu Objective 2 - Structured Lab Results >40% of all clinical lab test results ordered for reporting period results are either positive/negative or in numeric format Menu Objective 3 - Generate Patient List by Problem Generate lists of patients by specific conditions to use for quality improvement, reduction of disparities, research or outreach through the use of patient inquiry Menu Objective 4 - Generate Patient Reminders >20% of all patients 65 or older or 5 or younger sent appropriate reminder during the EHR reporting period Menu Objective 5 - Timely Access Provide patients with timely electronic access to their health information including lab results, problem list, medication list and medication allergies within four business days of the information being available to the provider Menu Objective 6 - Provide Patient Specific Education >10% of all patients seen are provided patient specific education resources Menu Objective 7 - Medication Reconciliation Perform medication reconciliation for >50% of patients transitioned into care of eligible provider by either importing CCR or CCD medical summary or manually compare a paper list, then record that reconcilliation was done Menu Objective 8 - Summary of Care Record Provide a summary of care record for >50% of transitions of care and referrals Menu Objective 9 - Submit to Immunization Registries Attest to capability of submitting electronic data to immunization registries or Immunization Information Systems & actual submission in accordance with applicable law and practice Menu Objective 10 - Submit Electronic Syndromic Surveillance Data to Public Health Agencies Capability to submit electronic syndromic surveillance data to public health agencies and actual submission in accordance with applicable law and practice. Perform at least one test to public health agencies.
