1. What is WoundRight?
The WoundRight application for the iPad and Android tablet is a comprehensive mobile wound, ostomy, and incontinence management system allowing care providers to spend more time on patient care and prevention; giving them the ability to accurately document wounds, perform detailed assessments, and completely manage treatments. With little implementation time and cost WoundRight offers true point of care treatment capturing complete, accurate documentation at the bedside.

2. Create Account
Navigate to www.woundrightapp.com via your tablet or computer to access the Create Account Page. ! Enter the required information and be sure to accurately enter the correct Challenge word ! View terms of service ! Click Create Account

Your account name will be your email address which will also be your User Name to log into the application. NOTE: You can also access the Create Account page from the login screen of the application. 2

3. WoundRight Server
The application and the server can send and receive information via syncing. Syncing takes place every time a user logs in and whenever the Sync now button is tapped form the Patient Directory page..

Every user needs an account. With an account you can have groups, manage patients, be affiliated with other accounts, have other accounts affiliated with you, etc. You can view you account information at a glance and also make changes: ! Click on Edit ! Make desired changes here ! Click submit Your account name will be your email address which will also be your User Name to log into the application.

Groups
You can add groups to an account; . A Group can be a wound care center, a hospital, or even key locations within a facility. Figure 3.3 shows the relationship between an account and its groups.

When an account is created that account name will be the first group. You can add a group from the new/edit patient pages.

Patients
A Group has a one-to-many relationship with a patient. As figure 3.5 shows each Group now has patients but the patients and Groups are all a part of the account holder.

To add a patient to a facility: ! Click Patients from the main AWP (Admin Web Page) screen ! Select Create New Patient ! You may choose to upload a photo ! Fill out form with relevant information ! Click Save when finished

Affiliates
An affiliate is any WoundRight account holder that is associated with another WoundRight account holder. An account holder can assign patients from within their Groups to an affiliate. The diagram below illustrates the theory behind affiliation with WoundRight. An account can be associated with other accounts, some could be hospital and some could be Wound Nurses, etc. An account can assign permissions to other account holders (affiliates) and they will then have access to those patients via the client (tablet).

Find Affiliates
As an account holder you can search for other account holders and send a request to become affiliated with them.

Simply start typing a name in the search box for an account holder or find them within the list and click Send Request. A request is asking an account to affiliate with you so you can share your patients and data with them. The requested account holder will have to accept your request before you can become affiliated.

Affiliate Permissions
Assigning permissions to an affiliate allows that account holder to gain access to all your patients information for the specific patients you have assigned to them.

Reports
You can generate reports from the Reports button located on the WoundRight Server home page. From there you can select to views reports for wounds, ostomy reports, and continence reports: ! ! ! ! ! ! Select a patient Choose a start and end date Choose which report you want to generate Check off what information you want included in your report For wounds: check off which wounds you want listed on your report Select Generate Report Figure 3.12

Wound Report

After generating a report you will be brought back to the Reports page. Here you can view the report you just generated, print it, or delete it.

Graphing
A graph can be generated from the Graphing section accessed through the the Server home page. The Area, Progress, and Dimensions of a wound can be graphed by ! Selecting a chart type ! If applicable selecting a wound ! Choose the start and end dates A graph with your specification will be generated.

Note: Choosing Area as your graph type will begin the animation of the graph because no other information is needed for that type.

Preferences
Configurations
Just as can be done from the app, some preference for the app can be changed from the Server. From within the Preference you will have the options to change Configurations; you can: ! Change the Diabetic Ulcer Classification system ! Select how to display patients ! Select to display patient photos

Uploads
Medications that Affect Healing
You can upload Medications via the Uploads section and clicking Medication that Affect Healing. On this page you can upload medications by: ! ! ! ! Clicking Choose File Locate your .csv file Select Choose Click Upload

You can view directions as well as download a sample file by clicking See Directions.

After your medications are loaded sync your device and you medications will be available Uploading Factors that Affect Healing and Pressure Redistribution Interventions can be accomplished by following the same steps as described for Medications. Also be sure to look at the directions for each and view the sample upload file.

4. Log in to WoundRight
To log in you must have a user name and password: ! If needed create an account by following the link as shown in figure 1.1 ! Type you user name and password and tap Log In If you forgot your password tap the link on the tile to reset your password. Each log in will sync the app with the server keeping WoundRight up-todate at all times.

5. Patient Directory Page

The Patient Directory page lists all the patients the current account holder has and any folders that may have been created. You can view other accounts patients by tapping the gear icon at the top right of the screen, then select the current account button. The selected accounts patients and data will be loaded and accessible. Selecting a folder opens that folder displaying the patients within. Tapping Up One Folder brings you a folder back, in this case to the Directory page. (Managing folders is discussed in the Resource Management section of this Manual).

6. Patient Management
Add New Patient
To add a new patient select New Patient from the patient directory page (figure 6.1). ! ! ! ! ! ! ! ! ! Fill in fist and last name Select gender Select date of birth Add photo (discussed in section 15) Select a Group Add a location Enter a Medical number Enter a Physician Select Submit

Edit Existing Patient


To edit a patient select from the patients directory page (figure 6.1). Follow same steps as above for the information you would like to change.

Inactivate a Patient
To remove a patient select from the patients directory page (figure 6.1). ! Tap Inactivate Patient (figure 6.3)

7. Wound Management
Add a Wound
To Add a wound select a body icon as indicated in figure 7.1. ! Rotate body part to desired section ! Tap location of wound on body Figure 7.2 shows the wound window

View Active Wounds


To view an Active wound, simply tap the wound on the body and it will be highlighted in the Active Wounds scrollable list or select it from the list.

Close a Wound
To close a wound, select the Close Wound button indicated on figure 7.1 ! Add comment (if desired) ! Submit The wound is available to open in Wound History which is discussed in section 14.

New Assessment
To perform an assessment on a wound first tap the Wound Management button (refer to appendix A). ! Tap New Assessment Figure 7.4 shows the assessment form. Complete form ! Within wound Bed Description: ! Adjust description values ! Add new description if needed If a pressure ulcer is added the assessment will change to accommodate other criteria for the assessment.

Description/Comments
Figure 7.8 shows the window to add a description or comment. To add description/comment simply tap in text field, enter desired text and tap submit.

Pressure Redistribution Interventions

To add an intervention to a wound select the yellow interventions button: ! First select a category ! Check off intervention(s) in each category ! submit Selections will be saved and displayed in the Selected Interventions section.

Factors Affecting Healing


To add a factor to a wound select the green factors button: ! Check off factors ! submit Selections will be saved and displayed in the Selected factors section. Note: Photographs are discussed in section 12.

8. Ostomy Management
Add a Stoma
Adding a stoma to a patient is simple. View figure 8.1. ! Rotate body from front or back ! Drag and drop stoma on body Figure 8.2 shows the stoma window

To view an Active stoma wound, simply tap the wound on the body and it will be highlighted in the Active Wounds scrollable list or select it from the list.

Close a Wound
To close a wound, select the Close Wound button indicated on figure 8.1 ! Add a note (if desired) ! Submit The wound is available to re-open in Wound History which is discussed in section 14.

New Ostomy Assessment


To perform an assessment on a stoma first tap the Ostomy Management button (refer to appendix A). The new assessment section will be selected by default. Figure 8.6 shows the assessment form. Complete form and select submit when finished. Description/Comments and Factors Affecting Healing are discussed in an earlier section on page 17. Photographs are discussed in section 12.

View Ostomy Assessment


To view an assessment select Assessment History from the main wound page (See appendix A). Tapping the assessment expands the tile for more details

Ostomy Treatments
Select Treatments from the main wound page ! Select either to add or load a treatment ! Add a treatment name ! Select a type ! Enter Manufacturer, Barrier, and Pouch ! Add accessories and treatments as necessary ! Submit treatment Remove a treatment via the Discontinue button.

12. Taking Photographs


Photographs can be taken for patients and wounds, for a patient Photo skip forward to Current patient, Otherwise consent needs to be given before a wound photograph can be taken: New Patient: Selecting to sign the form will display the consent form. ! Fill out the required information ! Have patient sign ! Have witness sign and enter name. ! Submit form Otherwise select form on file. Current Patient: To take a photograph: ! Select New Photograph button ! Tap anywhere on screen except Cancel ! to take picture ! Either select retake or save.

Resource & Configuration Management


From the Patient directory page (See appendix c) select Resource Management to view the Resource and Configuration Management menu (figure 16.1) Changes can only be made for the account currently logged. ** Please only use the Upload log Button when requested by support. **

Manage Patient Folders


Patients can be organized into folders. Select Manage Patient Folders to access the folders. ! The Main View is displayed first ! You can add a folder here or in any folder by tapping the button ! Folders can be added inside folders ! Simply tap the button to add a patient to a folder A folder can be deleted by first emptying that folder and then tapping the icon.
