Visual examination and Palpation: -Soft tissues: Lips: normal Mucosa: slightly red in color, but still normal. Tongue: shows no abnormalities. Palate: normal -Gingiva shows localized inflammation due to calcular deposits. Color: reddish especially on the lower gingival. Form & contour: there is observed gingival enlargement. Density: gingiva is observed to be soft upon palpation. Attachment: there is observed gingival recession. Sulcular depth:

-Teeth (crown): tooth no. 14 is root fragment, and tooth no. 16, 13, 12, 11, 21, 27, 47, 45, 44, and 35 are having caries. -Occlusion: the patient is class 1 type 3(anterior crossbite). SUPPLEMENTARY DIAGNOSIS Radiographs: -Teeth (root): tooth no. 16, 45, 42, 41, 31, 32, and 35 have an exposed root in which the patient is complaining tooth sensitivity after the supragingival scaling. -Surrounding structures: base on the radiograph of the patient there is vertical resorption of the bone but it is minimal only.

Diagnostics Cast: -contacts (lingual view): Class1 type 3(anterior crossbite) Mobility test: there is observed 1st degree tooth mobility on the tooth no. 31, 32, 41, 42 and 35. DIAGNOSIS: Mrs. Shella DC. Calugay a 38 years old patient suffering from a localized periodontitis, complaining with a foul breath and gingival bleeding every after tooth brushing and the gingiva is suffering from inflammation due to calcular deposits on her teeth.

Base on this Images the Patient shows to have a localized periodontitis. TREATMENT PLAN: Patient should undergo a 6 month periodontal treatment. This includes the scaling of her teeth every month. PROGNOSIS: The prognosis depends on the patients ability and desire to cooperate.
