Dear Nancy, We are pleased to extend you this offer of temporary employment with Veritude. Veritude is duly registered with the Massachusetts Department of Labor Standards as a Placement Agency. This letter confirms the details of your assignment. We anticipate that your period of temporary employment will be from February 7, 2013 until April 29, 2013, but this period may be shortened or extended by Veritude at its sole discretion. Any change to the duration of your assignment will not alter your status as a temporary employee, nor will it alter the atwill nature of your employment. Your assignment will be as a Communications Manager at PSG at Fidelity Investments Veritude. 155 Seaport Boulevard, Boston, MA. Your regular hours of work are 8:30am to 5:00pm Monday through Friday. When the need arises, you may be required to work additional, overtime or different hours. Your hourly rate of pay will be $90.00, less all appropriate withholding. Your position is eligible for overtime pay at Timeandahalf for hours of work you perform in excess of 40 in any given week. While employed at Veritude you must report to Veritude all workrelated accidents and illnesses within five days of occurrence. Veritude's Workers Compensation carrier is Broadspire. If you are a new Veritude employee, please refer to your Welcome Packet for time sensitive information regarding benefits for which you may be eligible. Please note, if you do not enroll online or via phone within the noted initial enrollment period, you will default to no coverage. Due to the nature of our business and as a condition of employment, we ask that you please sign and return to me the Employee Agreement enclosed in your Welcome Packet, which covers confidentiality, outside business activities, ethics, and other important matters. In addition, your Welcome Packet describes appropriate identification required to verify your employment eligibility, pursuant to the Immigration and Control Act of 1986. You must bring the appropriate identification with you on your first day of work. If you do not bring appropriate identification with you on your first day of work, your assignment start date may change and you will not be permitted to work until you provide the required identification to verify your employment eligibility. There are also other forms and documents in the Welcome Packet that you will need to complete and return to us on or before your first day of work. Veritude performs background security investigations of all new employees, including but not limited to drug screening, education, employment, reference, credit and criminal record checks and political contribution disclosure when appropriate. Completed political contribution forms are only required by candidates who are offered positions in one of Fidelity Investments Asset Management divisions (FMR Co., Pyramis and Asset Allocation Division). To comply with Fidelity's regulatory obligations, all employees working in Asset Management are required to disclose political contribution activity from March 14, 2011 to the present by completing a Political Contributions Disclosure Form. Any political contributions disclosed will be kept strictly confidential and will only be used for purposes of regulatory compliance and legally required recordkeeping and reporting. Our offer of employment is contingent on the approval of your political contribution history by Fidelity's Ethics Office. Employment and continued employment are contingent upon a satisfactory drug screening, positive references, political contribution disclosure, when appropriate, and background investigations, as judged by Veritude.
