ANNEX A: AGREEMENT ON COMPANY RULES AND REGULATIONS 1.

That when requesting for a long vacation or changes on personal schedule occurs Consultant should notify the company as soon as possible or one (1) week before the confirmed session. 2. That Consultant should fill-up the one-month schedule request on or before the 15th of each month. However, due to the nature of the work, it is understood that the desired schedule request may not always be the same with the approved schedule. 3. That Consultant is not allowed to make any changes in schedule one (1) week before the given schedule. Any alterations in schedule/s without the approval of the management shall be considered as gross negligence and shall be ground for termination. 4. That Consultant is fully responsible for checking or opening the given schedule seven (7) days prior to the said schedule. 5. That Consultant in case of emergency should notify the company six (6) hours before the schedule starts by calling Peter @ (02)442.20.89 Globe: 0915.180.21.55 ; 0905.326.29.36. 6. Failure to notify the office will mean salary deduction equivalent to the number of student reservation. Absence or tardiness of a Consultant without official leave on a confirmed schedule shall be considered as gross negligence and shall be ground for termination. 7. That absence without official leave for more than one (1) day of confirmed schedule shall be considered as abandonment of work and grounds for immediate termination. 8. That Consultant should file a Leave of Absence seven (7) days prior to the confirmed schedule and will only be considered binding when duly approved by the officer-incharge. 9. That Consultant who goes on official leave of absence and after its expiration, fails to return for work without reasonable or valid cause despite due notice of the expiry date of his leave without warning shall likewise be considered as having abandoned his work. (AWOL) 10. That Consultant is expected to report/login on time for the class. Salary deduction will be charged for tardiness. Consultant is required to login the official Skype ID 10 minutes before the first opened schedule/class and to remain logged in until end of schedule with or without student, unless permitted by the management to log out. For example, the Consultant opened the following classes 09:00, 09:30, 10:00, 10:30, and there is only a student for 10:00, the Consultant still needs to be online 10 mins before 09:00 regardless if there is a student or there is no student and needs to stay online unless permitted by the management to go offline. Moreover, to check the audio and video system as well as the connection with the officer-in-charge before the class starts. 11. That Consultant needs to refresh the reservation page every 5 minutes to check for student reservations.

12. That the Consultant is required to give a feedback for each of the student, during the first 2-3 mins. of the class. Student is online Student is still offline Student is online but not answering Student is absent Connection is choppy Call gets disconnected Audio problem, e.g. the student cant hear the teacher or vice-versa Drop call by the student Call failed Student didnt add the teacher to his/her Skype list

If the student is not on the Skype list yet, make sure to add the student 5 minutes before the class starts. If the students Skype ID cannot be found, ask the manager in-charge for assistance. If Consultant fails to give a feedback, class time will not be counted, no salary will be given for the said class and this can be grounds for termination. 13. That deliberately neglecting or refusing to obey legitimate orders, insolence and disrespect towards officials or other persons in authority is considered a serious offense and violation of Contract that shall constitute grounds for termination. 14. That by participating in, instigating, or leading the deliberate restraint and by interruption, disruption or stoppage of work a Consultant shall be terminated without any notice. 15. That inability to perform or neglect of duties in conformity with established standard of an average Consultant or minimum requirement of the company are grounds for termination. 16. That commission or omission of act or acts that provide reasonable ground or basis to show that this Consultant is not reliable or trustworthy is therefore deemed no longer worthy of continued employment. 17. That Consultant may not change their Skype ID password. 18. The Consultant may only use their Skype ID with _englishpower.jp during their official class time. 19. That Consultant is not in any way permitted to contact any of the clients of the company outside the official Skype ID, nor send any e-mail to any client without asking permission from the officer-in-charge. Violation shall immediately result to termination. 20. That Consultant terminating that Contract must give the company a 15-day grace period prior to the termination of Contract. And that Consultant may get the last salary only after one (1) month of the last working day. 21. That Consultant has no rights whatsoever to divulge to anyone outside of the company management any work related pertinent information such as students data and

information, lesson materials, etc. Such acts of business sabotage will not be tolerated by the management and is punishable under the E-Commerce Act of 2000. 22. That I have read all the contents of this Agreement and I understand fully the Agreement before I sign it. And that I am consciously abiding by this Agreement.
