I, parent or guardian of , hereby give approval for his/her participation in any and all Ashland Youth Football League activities during the current season. I assume all risks and hazards incidental to such participation, including transportation to and from the activities; and so hereby waive any and all claims against the Ashland Youth Football League program or its representatives on account of any accident or injury or other damage that may be incurred to on said players property in connection with, or incidental to, the above named program. I also grant permission to managing personnel or other league representatives to authorize and obtain medical care from any licensed physician, hospital or medical clinic should player become ill or injured while participating in league activities away from home when neither parent nor guardian is available to grant authorization for emergency treatment. **IMPORTANT INFORMATION & DATES TO REMEMBER**


Note from AYFL on what to expect: We put emphasis on teaching the game of football. The teams are balanced to keep each game competitive. We also put emphasis on good grades, and conduct in school. We want the players to learn how to work towards a common goal and reach it. They will learn to win like gentlemen, and lose like gentlemen. We are always reviewing our drills to insure safety at all times. We believe that every child is a better, stronger child, both physically and mentally at the end of the season. Because of the balance of our teams, we DO NOT move players around to accommodate car pools, friends, coaches, etc. We want you to know this going in. PLEASE MARK YOUR CALENDAR-May 18th is Equipment Fitting Day *Mandatory Equipment Fitting on 5/18/13 at Taft Elementary, 825 Smith Road, Ashland, Ohio from 10 a.m.-12 p.m. ALL PLAYERS MUST ATTEND sometime between 10 a.m.-12 p.m. The entire fitting process should take 30 minutes or less. For players who fail to attend the mandatory fitting equipment size and/or colors will not be guaranteed. *A representative from MVD Sports will be available to take orders for football shoes, if you are interested. *Football begins in early August with a telephone call from your coach. A Mandatory Equipment Pick-up will be held prior to first practice. Practices will begin shortly after the equipment pick-up day. Coaches determine their own practice schedule. During the summer each team may practice up to three times a weak. Once school starts, each team will only practice twice a week. Each player will need to wear gym shorts and a T-shirt. Please bring a water bottle to each practice. Due to the availability of fields, all games will be played on Sunday afternoons. Games are typically played at 1:00, 2:30 and 4:00. Players will receive a game schedule during one of their summer practices. *Refunds prior to 8/11/13 = $50 prior to 8/18/13 = $25 on or after 8/18/13 = NO REFUND. Although we would like every child
who registers to commit to the whole season, we know there are exceptions. Therefore, we have established this refund process.

*Head coaches are appointed by the AYFL Board. Each head coach may select three assistant coaches. The assistant coaches are also Board approved. It is our philosophy that the head coach must be able to teach, coach, and be a positive influence to each child on his team. No parents can be head coaches. ************************************************************************************************** *Community Donations have been an integral part of the ongoing success of the AYFL. The AYFL does not receive funding from the Ashland City or Hillsdale school districts. Donations have allowed the AYFL to keep registration fees unchanged at one of the lowest rates in the state.
