Ramin Tayani, M.D. is a board-certified, fellowship-trained ophthalmic surgeon, specializing in orbital-facial ophthalmic plastic and reconstructive surgery of the eyes and upper face. With over 12 years of experience, Dr. Tayani is one of Orange Countys leading experts in eyelid surgery. Dr. Tayani has chosen to specialize in this particularly delicate area of eye care to insure that patients with eye-related concerns receive the focused attention and specialized care they require.

Peter Joson, MD
Dr. Peter Joson is a Board-Certified ophthalmologist, fellowship-trained in glaucoma. His expertise in the diagnosis and management of general ophthalmic conditions and the many varieties of glaucoma result in optimal care. Dr. Joson believes that medicine, and particularly, ophthalmology and glaucoma, are both sciences and arts. He endeavors to apply the science of medicine and tailor a treatment plan to the individual patient.

Skin Resurfacing
Over time, factors such as sun damage, genetics, diet, and repetitive muscle movement can contribute to the formation of lines and wrinkles in the face. The development of pigmentation anomalies, such as age spots, or acne scars can cause skin to lose its smooth, youthful appearance. Botox is considered by many in the field of cosmetic medicine to be a wonder drug. Approved for cosmetic use in 2002 by the FDA, this treatment has been used therapeutically for over twenty-five years. Botox is a type of drug called a neuromodulator. Neuromodulators relax the muscle and prevent its activity. This helps slow the formation of fine lines and wrinkles. Botox is used cosmetically to achieve a more restful and smooth appearance of the areas treated. For cosmetic purposes, it is generally administered around and between the eyes and in the forehead. Botox is a quick procedure, taking no more than fifteen minutes and there is no downtime. The effects develop within three to five days and will typically last from three to six months. Occasionally, a touch-up may be required and will usually take place two weeks after the first treatment to ensure that the product has achieved its maximum benefit.

Facial Fillers
Like the name implies, fillers simply fill areas of soft tissue to flatten wrinkles, fill creases and create a full, youthful contour to the face. Injected fillers are also used to help fill out skin depressions and some types of scars. They are also frequently used to create a fuller, more lush appearance to the lip. The most common facial regions for treatment with injected fillers are the deep smile lines from the nose to the corners of the mouth (nasolabial folds), the lips, the circles under the lower eyelids, the cheeks, and the jaw line in front of jowls.

Fillers can also be used to reduce scars from acne or trauma. Injectables alone are not always sufficient for severe surface wrinkles on the face, or for the multiple vertical lines around the lips. A basic skin care routine is essential for optimal results. It is important to understand it is a 24 hour a day, 7 day a week regimen. There are several basic steps to follow. First, it is necessary to cleanse the skin morning and evening. Next you need to exfoliate; most people tend to skip this step in their weekly routine. This is the one step you will notice an immediate difference. You may use a scrub with small grains and gently scrub the dead skin cells from the surface of your skin. Do this on a weekly basis, but it is important to understand not to over exfoliate the skin.

Another skincare rule is to use a good moisturizer, even if your skin is oily. Apply the moisturizer to your skin while it is still moist. Finally, apply sunscreen. Never leave home without it! Eyelid surgery is performed in an outpatient surgery center under general anesthesia or a combination of local anesthesia and sedation. The procedure provides permanent results, and it is rare for someone to need a secondary blepharoplasty.

The eyes are one of the first parts of our face to show aging. Excess skin, caused by a natural decrease in skin laxity, can cause a droopy upper eyelid or a wrinkled, crpey appearance to the lower eye area. Women experiencing this often say they have difficulties putting on mascara and other eye make-up. This concern can be remedied through a surgical procedure called blepharoplasty. Often referred to as eyelid surgery, nearly 150,000 procedures were performed in the US last year. Facelift
As we age our skin loses elasticity, the facial tissues lose their youthful fullness and the supporting structures starts to sag. This process begins in our thirties. Facial fillers can provide some temporary relief and for some is sufficient to achieve their aesthetic goals. However, repositioning the fallen underlying structures responsible for the descent of the cheeks, sagging jawlines, and a blunted neck is best managed by performing a facelift. The goals of a facelift include the elevation and resuspension of underlying tissues as well as restoring the volume of the upper face. Incisions are placed as inconspicuously as possible, in the skin folds in front of the ear, the ear canal, behind the ear and in the crease beneath the chin. Otoplasty
Otoplasty is defined as an operation to reshape the ear. Common aesthetic concerns regarding the ear are prominent ears and/or absence of the cartilage fold close to the top of the ear referred to as the antihelical fold. Most otoplasty procedures involve excision of a wedge of cartilage from the concha, the bowl shaped cartilage. The surgery is performed through an incision within the fold on the back of the ear. This allows the ear to be rotated back closer to the head relieving the prominent appearance. If necessary, using suture techniques, an antihelical fold can be created. The procedure may be performed under local anesthesia with sedation.

Rhinoplasty
Rhinoplasty is defined as surgery to reshape the nose. Nasal deformity may be purely cosmetic in nature or as a result of trauma. As one of our most prominent facial features, the nose is a focal point of attention. Common complaints include a hump that interrupts the smooth contour, an overly prominent nose, a drooping tip, a tip that is too wide or bulbous, poor tip definition, and breathing difficulties. A chin that is too small creates a loss of facial harmony and makes the nose look even larger than it is. For this reason many rhinoplasty patients benefit from a simultaneous chin augmentation. Most rhinoplasties are performed with all incisions hidden inside the nose (closed rhinoplasty). Dr. Citron is an expert in rhinoplasty using the closed technique and nasal reconstruction.

Neck Lift (Cervicoplasty)


Aging of the neck is a result of multiple processes including sagging of the muscles, accumulation and decent of fat and loss of soft tissue. A tendency toward accelerated change is more pronounced in people with relatively smaller chins, permitting additional sagging to be present. Neck lifts are an integral part of face lifting, but in selected individuals may provide adequate correction alone. Techniques are customized to the patients specific needs and goals. Liposuction, muscle suspension, chin augmentation and excess skin excision are used to recreate the youthful contours of the neck. Many procedures are performed in a minimally invasive fashion with minimal downtime.

Brachioplasty
Brachioplasty or surgery to reshape the arms is performed to relieve the excess skin and fat that hangs. Traditional brachioplasty is performed using a combination of skin excision and liposuction. For a large amount of hanging skin, this is the only method that will give a satisfactory result. However a large scar extends on the back of the arm, much like a seam on a shirt, from the elbow to the armpit. For those with a lesser amount of sagging skin, as the patient shown to the right, laser-assisted liposuction can be used to achieve removal of fat and skin tightening without the large scar. Recovery is enhanced by wearing an elastic sleeve to help reshape the arm and limit swelling. Dr. Citron was one of the first plastic surgeons to develop this technique and has helped many women have better shaped arms without the scars of traditional brachioplasty.

Liposuction
Liposuction or Suction-Assisted Lipectomy is a procedure that sculpts the body by removing unwanted fat from specific areas. Commonly requested areas include the neck, chest, abdomen, back, arms and thighs. Using specialized techniques of laser-assisted liposuction and ultrasonic liposuction problem areas such as the male chest, upper back, bra line and loose skin can be effectively treated. All treatments involve the use of tumescent technique in which a saline solution containing an anesthetic agent is infiltrated into the treatment area prior to liposuction to facilitate fat removal, decrease bleeding and increase patient comfort. Dr. Citron is an expert in liposuction and body contouring who has been performing liposuction for nearly 20 years, developed and taught techniques of laser-assisted liposuction for Sciton Laser Corporation.

Autologous Fat Transfer


As we age there is a loss of youthful fullness of the face. In some patients the predominant mechanism of aging is soft tissue loss. Restoration of volume with fat is a natural alternative to fillers for restoring volume of the cheeks, lips and hands as well as many other areas of the body. Fat is harvested from a small incision on the abdomen, purified and injected using small syringes and specialized blunt tipped needles. Autologous Fat Transfer is commonly performed with facelift surgery to restore volume in the perioral creases and is an important component of hand rejuvenation.

Breast Augmentation
Breast augmentation can be performed to fulfill your desire for fuller breasts or to restore breast volume lost after pregnancy or weight loss. Dr. Citron believes that breast augmentation should be performed with the goal of achieving the most natural appearing result. Choosing the proper implant size is done in consultation after discussing your aesthetic goals, examining and taking into consideration the dimensions of your chest and skin envelope. Most patients choose silicone gel implants, however saline-filled implants are still a good choice for many. The decision to undergo breast augmentation surgery is a highly personal one as breast augmentation not only improves ones silhouette but also may greatly enhance self-esteem.

Breast Lift (Mastopexy)


Pregnancy, nursing and the relentless pull of gravity can take its toll on the female breast. Loss of breast volume and skin elasticity can cause the breasts to sag. A mastopexy or breast lift is a surgical procedure that raises and reshapes the falling breast. A small degree of sagging may be corrected by excising skin around the areolae resulting in a circumferential scar around the nipple-areolar complex. More extensive sagging requires a vertical incision from the areola to the base of the breast to remove excess skin. Severe sagging is treated by the standard approach that utilizes an incision within the fold underneath the breasts well resulting in an anchor scar to excise the redundant skin. Many women with sagging breasts also have significant volume loss and desire correction requiring simultaneous augmentation. Augmentation mastopexy, as seen in the patient shown, is a complex procedure, regarded by many plastic surgeons as one of the most challenging, that essentially decreases the size of the skin envelope and increases the size of the breast simultaneously.

Abdominoplasty
In many women, pregnancy stretches the abdominal wall and skin resulting in loss of elasticity and skin sagging. Abdominoplasty or reshaping of the abdominal wall, commonly referred to as a tummy tuck, is a surgical procedure that removes redundant skin and fat from the abdomen and tightens the muscles of the inner abdominal wall. When performed as an outpatient procedure, the slender contours of the youthful abdomen can be restored in hours. Liposuction of the flanks and lifting of the lateral thigh skin can be done at the same surgical setting. Patients with a lesser degree of bulging and skin excess may be candidates for a smaller procedure or Mini-Abdominoplasty. A supporting garment is worn for 4-6 weeks after surgery for added comfort and to hasten recovery.
