- Hospital Kemaman (District), Trengganu, Malaysia : 3 months, 1988. I worked in the


capacity of a Medical Officer handling all medical problems, in elective and emergency
situations including Obstetrics practice. Emergency practice include a heavy load of
Trauma cases. I liaised quite regularly with the consultants of various disciplines in
Hospitals Kuala Trengganu and Kuantan. During this stint I did house calls for
emergency obstetrics cases and also standby calls for VIPs and dignitaries visiting the
area.

- Hospital Universiti Sains Malaysia, Kelantan, Malaysia (1.5 Years In General Surgery) :1988-1990. I
worked as a Medical Officer / junior Trainee Lecturer (Trainee Surgeon) in a Dept consisting of five
general surgeons and one neurosurgeon. I started teaching the senior medical students on clinical surgery
during my business ward rounds at this stage.

The final year was focused in general surgery and I functioned as a registrar directly
under supervisioned of a team of five senior general surgeons

d. Visiting Surgical Specialist Registrar At The Hepatobiliary And Pancreatic


Unit(University Dept. Of Surgery), The Royal Infirmary, Edinburgh (14.8.1995 -
31.1.1996). This is the premier Hepatobiliary Centre in Scotland which also houses the
only Liver Transplant Centre in that country. I was directly responsible to the consultants
of the unit during elective work but also answerable to many other consultants from other
general surgery units during emergency services. I was also an ad-hoc member (Surgeon
Assistant) of the Liver Retrieval Team for the Scottish Liver Transplant Unit, The Royal
Infirmary of Edinburgh, UK, 1995 – 1996. I had assisted in retrieving 10 complete liver
grafts from cadaveric donors from hospitals all over Scotland.

e. Visiting Surgical Specialist Registrar In The Minimal Access/Upper


Gastrointestinal Unit At The Department Of General Surgery, Western General Hospital,
Edinburgh, UK, from 1.2.1996 - 31.7.1996. This second major hospital in Edinburgh
houses the most prestigeous minimal access surgery unit in Scotland and I was directly
responsible to two senior consultants in my elective work and four during my emergency
services. This hospital also houses the only Breast Unit in Scotland and I was exposed to
their clinical activities and also covered the unit for emergency services.

1977. This scholarship was given to five top students in the school based
on the overall lower secondary school academic performance culminating

IV. Public Service Department Scholarship From The Government Of


Malaysia For Undergraduate Studies in Australia, 1978 – 1985. This
scholarship was given based on the consistent excellent academic
performance in Forms 4 and 5 prior to the results of the MCE
examinations.

VI. Higher Academic Staff Scholarship From Universiti Sains Malaysia For

VII. Association Of Endoscopic Surgeons Of

(£1000.00). This annual travelling scholarship was granted to about 5
advanced surgical trainees or junior consultant surgeons in the whole of
UK and Ireland. The purpose of the scholarship was to enable the holders
to travel to the centres of excellence in Endoscopic Surgery in the UK,
Europe or USA

Clinical (Surgical) Committment In USM - Malaysia

I. About 6 days On-Calls Per Month In Specialist(Consultant) Capacity for General


Surgery. This involves receiving new emergency patients, managing inpatients at
nights and also performance of emergency operations at any time of the day.

II. Once - a - Fortnight Outpatient Surgical Clinic Running One Full Morning (4
hours) – concomitant Senior Medical students (4th and 5th Year) and Postgraduate
teachings

III. Regular Ward Rounds for in-patients management – twice a week (2 hours each
– concomitant Senior Medical students (4 th and 5th Year) and Postgraduate
teachings

IV. One Operating Day Per Week (8 hours) – concomitant Senior Medical students
(4th and 5th Year) and Postgraduate teachings

V. I am a crucial member of the Gastroenterology Team in Hospital USM.


Gastroenterology service is well established in HUSM for more than a decade
and this unit has been recognised by the Ministry of Health as an accredited

training centre for Gastroenterology and Endoscopy. Decision making on
complex gastrointestinal problems that require both medical and surgical input
are often made after mutual discussion with the team members.

VI. Provide Support/Cover For Neurosurgical Services In Elective And Emergency


Situations. This is the only Neurosurgical Unit in the East Coast of Peninsula
Malaysia which handles a very heavy clinical load of neurotrauma.
Neurosurgical Unit in the Dept of Surgery is the central component of the newly
established Neuroscience Unit in the School of Medical Sciences, USM.
Examples of Combined clinical cases : Anterior approaches to the cervical and
thoracic spines in cases of thyroid cancers and tuberculosis and Laparoscopic
approach to salvage VP shunt

VII. Provide Support/Cover For Urological Services In Emergency Situations.

VIII. Provide support for Otorhinolaryngology services in elective situations. This


contribution essentially involves the performance of “Stomach pull up”
procedures as a pedicle graft for reconstruction of gastrointestinal tract after a
radical pharyngolaryngectomy.

IX. Provide support for Gynaecology services in elective and emergency situations.
This participation mainly congregates on the common interest of utilizing
minimal access approach to manage abdominal diseases by way of laparoscopy –
diagnostic and therapeutic

X. Provide support for Orthopaedic and Plastic Surgery services (Tumour and
Reconstructive Team) in elective situations. The Tumour and Reconstructive
Team of HUSM is perhaps the only such team in the country performing major
ablative surgery on the musculoskeletal system with plastic reconstruction to
restore structures and functions. Examples of participations : combined approach
in thoracic and abdominal wall surgery, pulmonary metastasectomy of soft tissue
secondaries following Limb-salvage surgery

XI. Hospital USM has recently officialised the formation of Combined Surgical-
Oncology Clinic. HUSM is the only Oncology-Radiotherapy Centre in the East-
coast of Peninsula Malaysia and hence a Tertiary Refferal Centre for all cancer
patients that require multimodal therapy. In line with my interest in Digestive
Surgical Oncology, I am a crucial member of this team.

Special Procedures and Operations Performed (USM - Malaysia) :

1. Diagnostic and Therapeutic, Upper and Lower Gastrointestinal Endoscopy.


Weekly elective schedule and Emergency services. I perform diagnostic upper
endoscopy with a prospect of starting Chromoendoscopy in routine practice soon.
As emergency therapy, I conduct sclerosant injection for bleeding oesophageal
varices and adrenaline injection for bleeding peptic ulcers. Other therapeutic
procedures : foreign body removal, Percutaneous Endoscopic Gastrostomy
(PEG) insertion, snare polypectomy (during upper and lower endoscopy)

2. Diagnostic and Therapeutic, Endoscopic Retrograde Cholangiopancreatography.
Weekly routine schedule and Emergency services. Diagnostic procedures include
biopsies and exfoliative cytology. Therapeutic procedures : papillotomy, stone
lithotripsy, endoprosthesis and exoprosthesis insertion and foreign body removal

3. Routine Laparoscopic Cholecystectomy

4. Selective Laparoscopic Appendectomy

5. Selective Laparoscopic Hernioplasty (TAPP)

6. Routine Diagnostic / Staging Laparoscopy for gastrointestinal tumours

7. Choledochoscopy for difficult biliary surgery

8. Thoracoscopy (VATS - Video-Assisted Thoracic Surgery) - Diagnostic and


Therapeutic. Pulmonary wedge biopsy for diagnosis and pulmonary
metastasectomy for resectable soft tissue tumour secondaries. I also regularly
utilize Stereotactic CT localization with the new Helical CT scan prior to VATS.

9. Thoracotomy – for Non-cardiac Thoracic surgery and Anterior Access for Spinal
Surgery

10. Radical Oesophagectomy. Two-field and Three-field Lymphadenectomy in


oesophagectomy for resectable Oesophageal cancers.

11. D2 and D3 Lymphadenectomy in Gastrectomy for resectable Stomach cancers

12. Liver resections of varying degrees. Although there is no case suitable for formal
segmental resection thus far, I have been using the Ultrasonic Dissector in
performing hepatotomy and minor non-segmental resections.

13. Bilio-digestive bypasses of various complexities.

17. Lichtenstein Hernioplasty for Inguinal Herniae. I have popularized this


universally accepted gold standard procedure in the treatment of adult hernias in
HUSM. HUSM is perhaps one of the few major hospitals in the country that
performs this operation routinely.

18. Routine use of Staplers in the performance of anastomosis in open surgery and some
laparoscopic procedures.

19. The use of Harmonic Scalpel in laparoscopic and open surgery. Harmonic Scalpel is
a newly developed technology in power source for surgery.

20. The use of Cryosurgery in the management of advanced tumours of the liver

21. The use of Radiofrequency ablation in the management of unresectable liver tumours
