Final Evaluation For Logan Franklin

To be completed by practicum supervisor and posted by the student on BB This form is to be filled out by the Practicum Supervisor and administered to the practicum student during Week 14 (or close to the end of the semester). Both supervisor and student should sign the form and an original copy must be sent to the Practicum Coordinator by the end of the last week of classes so a final grade can be posted for EAD 747 (Practicum class). The student has completed the objectives defined for the practicum experience to the following degree: (Please rate using the following scale, comments are optional) 1 = Development Needed 2 = Good 3 = Very Good 4 = Excellent

3: Learning Objective 1: Understand the purpose and function of the First-Year Programs office. Comments: We have discussed a variety of topics regarding the office operations throughout the semester5. I believe Logan has a fairly good idea of our primary purposes in FYP. 2 Learning Objective 2: Develop skills in building relationships with students . Comments: I agree with Logans self-evaluation observations that his 1:1 meetings with students have gone well. In fact, I think he is at his best when engaging with students individually in a structured setting. As he mentioned in his self-eval, an area in which he seems to struggle a bit is engaging students casually in conversation prior to the start of classes. 2 Learning Objective 3: Build a mentor relationship with the students. Comments: Similar comments apply to this similar learning objective. Several students have completed 1:1 meetings, and Logan was able to easily articulate common themes of concern from the students.

3: Learning Objective 4: Enhance my Student Learning and Development Competency by learning to construct a plan for teaching students a topic. Comments: Logan developed a lesson plan/case study/discussion approach, and he created plans for several partial class periods in GEP 101 with his lead instructor. 3: Learning Objective 5: Enhance my leadership development skills with the Peer Leader class. Comments: Logan prepared a couple of effective lessons for the Peer Leader classes this semester. He developed a good class discussion exercise regarding ethics and multicultural issues; these are two areas in which PL sometimes struggle a bit addressing new students.

Please use the same rating scale as above and rate the students performance in the following areas--(comments optional): 3 Professional attitude, behavior, and relationship with Practicum Supervisor Although Logan started the semester with some challenges on being on time, that improved greatly later in the semester. Overall, Logan displays an appropriate professional attitude and communicates well with the office. He does a good job emailing me with any questions, or to confirm meeting times. 4 Professional relationships with others in the office or area, colleagues, administrative staff 4 Effective communication (oral, written reports, email, project presentations) 3 Ability to adjust to changes in projects or tasks, effectiveness in completing tasks 3 New professional skills acquired Recommended Grade: A
