Learning at Your Fingertips
This year is an exciting time for AABB education
it brings a doubling downof commitment to expert educational resources for
our community. In addition to expanding an ever-richer portfolio of content,
this is also a year of investment in new technology platforms that will deliver
professional development tools to support all learning styles and schedules.
The expanded content in transfusion medicine, patient blood management and
cellular therapies provides the information needed by facilities to improve the
quality of care that they are delivering to patients and donors. In the cellular
therapy arena, standards for the laboratory setting have been expanded into
the clinical care setting. Those faced with regulatory challenges now have access
to a toolkit that provides customizable templates for regulatory submissions.
For those facilities implementing patient blood management programs, multi-
media learning modules will be available to provide the foundational knowledge
to implement your program or to take your existing program to the next level.
The already comprehensive library of print and electronic publications is being
further supplemented with forward-thinking transfusion medicine content such
as Antibody Identifcation: Art or Science? A Case Study Approach, as well as new
editions of invaluable references such as The Transfusion Committee: Putting
Patient Safety First.
In parallel with expanding our collection of relevant content, we are revamping
our delivery platforms to make it easier to access that content. An improved online
Marketplace will bring together all modes of learningfrom webinars to books
to digital toolkitsmaking it easier to nd resources that t your needs. A new,
robust learning portal will allow you to manage all of your career development
progress in one place.
Yet, amid all of these exciting changes, one thing holds constantthe
contribution of leaders in the eld who collaborate to develop these innovative
resources. Only through the deep partnership with our professional volunteer
experts are we able to bring this body of knowledge to the eld.
I hope you will take full advantage of what AABB has to oer. Arming yourself
with leading-edge knowledge is good for your professional growth, good for
your facility, and good for advancing the eld.

Romancing the Big Q:
Dancing with
the Quality Gorilla
Edited by Lisa M. Walters, PhD, MBA, MT(ASCP)SBB
Quality systems have long been
indispensable to the function of
health-care institutions. The value in an
institutions relationship with quality is
uncovered when it is allowed to evolve.
Committing to a culture of continual
improvement can be something like dancing with a gorilla who never
tires; it means developing the skills of adapting to changing demands and
environmentscyclically.
Building on material from Introducing the Big Q: A Practical Quality Primer
(AABB Press, 2004), Romancing the Big Q explains how to move an
organization toward performance excellence. Each chapter is half theory,
half storyrst laying out the quality concepts, and then illustrating them
in a practical case study.
Topics include:
n Strategic planning with matrices.
n Vision and goal setting.
n Solidifying leadership.
n Process management and plan deployment.
n Measurement and evaluation.
n Risk management.

Antibody Identication:
Art or Science?
A Case Study Approach
By Janis R. Hamilton, MS, MT(ASCP)SBB;
Susan T. Johnson, MSTM, MT(ASCP)SBB; and
Sally V. Rudmann, PhD, MT(ASCP)SBB
This book steps in where hands-on
practice may struggle to go. Written by practicing serologists and educators,
these case study simulations examine techniques for alloantibody identication
including use of chemicals, inhibition, adsorption, and adsorption/elution. Each
case begins with a clinical scenario and initial test results, which are followed
by a series of multiple-choice questions that oer testing options and protocols
for resolution. Along the way, the reader is provided with detailed feedback
designed to enhance reection and critical thinking. Equally suited to classroom
or individual study, the printed book is supplemented by an online component
without the answers, to provide a realistic testing situation.

Guidelines for Antibody
Identication
Developed for the Scientifc Section Coordinating
Committee by Marilyn Moulds, MT(ASCP)SBB,
and Mary Kowalski, MT(ASCP)SBB
This Guideline is intended to assist transfusion
services with the identication of antibodies in
patients with a reactive pretransfusion antibody
detection test. Its major sections address
1) routine testing and interpretation guidelines,
2) additional guidance and testing, and 3) unusual antibody identication
situations. Instructive case studies accompany each section, and more guidance
is included in several appendices. Although this resource can be used as a training
tool, those readers who are already familiar with related material in the AABB
Technical Manual will derive the most benet. The scope and depth of the content
will appeal to facilities and technologists who work with a single antibody
identication panel using the same method employed for antibody detection,
as well as laboratories that use multiple panels and special testing methods.

Special Situations Tissue Management
Guidelines for Transfusion
of Erythrocytes Containing
Sickle Hemoglobin
Developed for the Scientifc Section Coordinating
Committee by Jerald E. Mullersman, MD, PhD, and
Gary E. Carnahan, MD, PhD
Circumstances under which blood from donors
with sickle cell trait should be avoided have not
been clearly delineated because evidence from
controlled studies is lacking. AABB standards require each transfusion service
to establish a policy that describes when to avoid transfusion of sickle cell trait
erythrocytes. This Guideline reviews the various recommendations being made
in the absence of controlled studies and critically examines the basis for them.

Guidelines for
Massive Transfusion
Developed for the Scientifc Section Coordinating
Committee by Arthur Bracey, MD
The safety and adequacy of blood used in
massive transfusion presents a challenge for
transfusion services. The Guideline addresses
how a transfusion service can play an important
role by designing and implementing systems
to eectively manage patients with massive transfusion. The rst part
of this Guideline, Medical Considerations, addresses hemotherapy
management options for patients receiving massive transfusion.
These options take into account available studies and expert position
statements on the subject. The second part, Laboratory Considerations,
discusses issues surrounding the design and implementation of a massive
transfusion protocol.

Guidelines for Transfusion
of Erythrocytes Containing
Sickle Hemoglobin
Developed for the Scientifc Section Coordinating
Committee by Jerald E. Mullersman, MD, PhD, and
Gary E. Carnahan, MD, PhD
Circumstances under which blood from donors
with sickle cell trait should be avoided have not
been clearly delineated because evidence from
controlled studies is lacking. AABB standards require each transfusion service
to establish a policy that describes when to avoid transfusion of sickle cell trait
erythrocytes. This Guideline reviews the various recommendations being made
in the absence of controlled studies and critically examines the basis for them.

An Eective Tool for
Improving Transfusion Safety
Edited by Ren R. P. De Vries, MD,
and Jean-Claude Faber, MD
Hemovigilance is a quality processthat aims
to improve quality and increase safety of
blood transfusion by surveying all activities
of the blood transfusion chain, from donors
to recipients. This volume is above all a
practical guide to setting up and improving hemovigilance systems, while
raising awareness for reporting adverse events and reactions. This is the rst
international book on hemovigilance, assembling all the vital issues in one
denitive reference sourceessential reading for all sta involved in the
transfusion process.
A general introduction includes chapters on hemovigilance as a quality tool for
transfusion as well as concepts of and models for hemovigilance. The core of
the book describes how hemovigilance systems have been set up and how they
work in hospitals, in blood establishments and at a national level. Chapters on
hemovigilance at the international level, achievements and new developments
complete the picture.

Guidelines for the Laboratory
Evaluation of Transfusion
Reactions
Developed for the Scientifc Section Coordinating
Committee by Robertson Davenport, MD
While infectious transfusion complications
are often the focus of patients, physicians,
blood transfusion professionals and regulatory
agencies, nonviral and immunologically based
complications are the most prevalent risks for a transfusion recipient. The
diagnosis and evaluation of any transfusion reaction is a matter of clinical
judgment by a physician trained in the adverse eects of transfusion.
Negative testing results should not be interpreted as denitely excluding a
transfusion reaction if there is a strong clinical suspicion that the patients
symptoms are related to blood transfusion. The laboratory evaluation
of suspected transfusion reactions is critical not only to the health and
well-being of the patient but also to the process of improvement through
corrective and preventive actions.

Guidelines for Pneumatic
Tube Delivery Systems:
Validation and Use to Transport
Blood Components
Developed for the Scientifc Section
Coordinating Committee
by Monique Mohammed, MS, MT(ASCP)SBB;
Roger Richard, MS, MT(AMT): and Lynne Uhl, MD
Many hospitals and transfusion services are
now using pneumatic tube delivery systems as a means of delivering blood
products faster and more eciently. To ensure product safety and the
maintenance of quality, system validation is key. The goal of this Guideline is
to provide direction when considering pneumatic tube technology for blood
component distribution. For facilities currently using this technology, this
serves as a comparison between legacy methods and those addressed by the
current validation. For institutions using legacy pneumatic tube systems, this
document can help direct procedures needed to maintain long-term quality
assurance goals.

AABB Interorganizational Task Force on
Domestic Disasters and Acts of Terrorism, this handbook assists community
blood centers, hospital blood banks and transfusion services in preparing
for and responding to domestic disasters and national acts of terrorism that
aect the blood supply.
The handbook contains two major areas of focus:
n Preparation strategies to implement before an event.
n Activation procedures once an event has occurred.
Included as an addition to the handbook is the Disaster Operations Handbook
Hospital Supplement prepared especially for transfusion services and for
hospitals that do not collect allogeneic blood. The supplement will help
facilities prepare for their role in providing blood for transfusion in
the event of a disaster. Instructions for working with local
blood centers to assess the eects of a disaster on the
local blood supply are also provided.

Guidelines for Donor
Hemoglobin Determination
Developed for the Scientifc Section Coordinating
Committee by Ritchard G. Cable, MD, and
Julie L. Houseworth, MD
Anemia in prospective blood donors may
indicate an underlying disease, may result in an
ineective blood product and may cause harm
to the recipient. Thus, the measurement of donor
hemoglobin as a means of determining donor eligibility is of paramount
importance. This Guideline identies several of the methods currently in use
for determining hemoglobin levels. The advantages, disadvantages, technical
aspects and quality assurance considerations of each method are described
so that collection personnel can choose a particular method that ensures
protection of the donor and a safe and eective product for the recipient.
