MANIFESTATION TO SERVE AS A VOLUNTEER SUMMER
KINDERGARTEN TEACHER

In the spirit of volunteerism, and with the desire to help the
Department of Education, particularly the Division of Naga City, in realizing
the objectives of the Education for All (EFA) and aims of the Universal
Kindergarten Education, I_____________________________________, of
______________________________________, voluntary offer my
services as Kindergarten Teacher at
__________________________________School, Naga City Division, this
Summer 2012, under the Kindergarten Summer Program (KSP) of the
Department of Education.

I understand that as a volunteer kindergarten teacher, I will receive
an allowance of Three Thousand Pesos (Php. 3,000.00) monthly for one
class of 20 - 30 pupils or Six Thousand Pesos (Php. 6,000.00) for 2 classes
per DepED Order No. 21, s. 2012.

I will willingly perform the tasks expected of me by the Department of
Education, Division of Naga City as a volunteer kindergarten teacher. My
application as a kinder teacher in the school stated above was done by me
voluntarily, and that I was not coerced by anyone.
