LIABILITY RELEASE FORM

Release of All Claims

In consideration for being accepted by the Fox Valley Childrens Chorus for participation in one of its
choirs, we (I), do for ourselves (myself), and for and on behalf of my child-participant do hereby
release, forever discharge and agree to hold harmless the Fox Valley Childrens Chorus, its directors,
officers, employees, agents and successors and assigns thereof from any and all liability, claims, or
demands for personal injury, sickness, or death, as well as property damage and expenses, of any
nature whatsoever which may be incurred by the undersigned and/or the child-participant that
occur while said child is participating in any activities of the Fox Valley Childrens Chorus.

In connection with the above to release the Fox Valley Childrens Chorus (along with its director,
officers, employees, agents, and successors and assigns) from any and all liability and claims, we (I),
the parent(s) or legal guardians of the child-participant agree to be solely responsible for
transportation of the child-participant to and from choir rehearsals or choir concerts, and we (I)
agree to be solely responsible for supervision of the child-participant both before and after any choir
rehearsals or concerts. The Fox Valley Childrens Chorus shall not be responsible in supervising the
drop-off or pick-up of child-participants. We (I), the parent(s) or legal guardian of the child-
participant agree to be solely responsible to make sure that the child-participant is properly dropped
off or picked up at any rehearsals or concerts.

The undersigned further hereby agrees to hold harmless and indemnify said Fox Valley Childrens
Chorus, its directors and officers, employees and agents, for any liability sustained by said Fox Valley
Childrens Chorus as the result of the negligent, willful or intentional acts of the child-participant,
including expenses incurred attendant thereto.
