To vote
To vote for a candidate for Chief Justice of the
Supreme Court; Associate Justice of the Supreme
Court; Presiding Justice, Court of Appeal; or
Associate Justice, Court of Appeal, connect the
head and tail of the arrow after the word YES. To
vote against that candidate, connect the head and
tail of the arrow after the word NO.
To vote for a candidate connect the arrow pointing
to your choice with a single line.
To vote on any measure, connect the arrow
pointing to the word YES or the word NO.
Important:
Check both the front and the back of your ballot
for voting contests.

Top-Two Open Primary
Under the Top-Two Candidates Open Primary Act, approved by the voters in 2010, only the two candidates
who received the top number of votes in the primary election for 8

Congressional, 8

State Senate, 26
th
State
Assembly districts and all state constitutional offices will appear on the General Election Ballot. Candidates for
these offices may have the same political party preference. Additionally, write-in candidates are no longer
allowed for these contests at the General Election. For more information you may contact the Inyo County
Elections Department at (760) 878-0224.

The political party preference of candidates will show on the ballot in the following ways:
Candidate has a political party preference: Party Preference: (name of party)
Candidate is registered with no political party preference: Party Preference: None
Abbreviations will be used to designate political party preference if the full name cannot fit on the
ballot.
Party Endorsements
Proposition 14, also authorizes qualified political parties to endorse candidates for voter-nominated
offices in the Sample Ballot booklet. The following lists of candidates are the official endorsements
that were received by the deadline. The endorsements listed may be from a different political party
than the candidate prefers. Please refer to the official ballot to view the candidates political party

PAUL COOK
Occupation: United States Representative
Education and Qualifications: Two years ago, as a retired COLONEL
with 26 years of service in the US Marine Corps, I asked for your
trust to become your Congressman. Having been awarded the Bronze
Star and two Purple Hearts for my time in Vietnam, I witnessed great
sacrifices by my fellow Americans and pledged to ensure their efforts
would not be in vain.
As your Congressman, Ive honored that commitment by fighting for
a balanced budget, for our veterans, to keep jobs in America, and to
defend our way of life.
I fought to pass a balanced budget plan to reduce the deficit by $4.6
trillion over the next ten years, strengthen Americas economy, and
protect Medicare for seniors. I voted over ten times to defund, delay,
and replace Obamacare.

Education and Qualifications: As a fourth generation Central Valley
farmer and independent businessman, Im proud so many community
leaders and respected organizations want me re-elected to the State
Senate. A few examples: Im endorsed by Fresno County Sheriff Margaret
Mims, the California Grape & Tree Fruit League President Barry Bidwell
and CDF Firefighters, the largest fire department in the state.
During my service, I wrote legislation that would end environmental
regulatory abuses and stop frivolous lawsuits that are costing our Valley

Education and Qualifications: I have served as a classified
employee, classroom teacher, and administrator during my 31 years
in education and therefore have a strong understanding of the
challenges facing educators. Over fifteen years of this experience
was gained in the Lone Pine Unified Schools so I am aware of its
unique character and needs. As a parent of three and grandparent
of two, I have personal knowledge of parental concerns and priorities.
I strongly support communication between home and school.
If elected, I will work toward developing a strong, cohesive team
that will support our administration and staff in the utilization and
balance of resources to ensure that all of our students become
productive citizens and competitive potential employees in whatever
paths they choose. I believe that we have a particular responsibility

AN ORDINANCE OF THE LONE PINE FIRE PROTECTION
DISTRICT IMPOSING A SPECIAL TAX, FOR EMERGENCY
RESPONSE SERVICES, UPON PROPERTY
WITHIN THE DISTRICT
The Governing Board of the Lone Pine Fire Protection District of
Inyo County ordains as follows:
SECTION I. TITLE AND APPLICATION.
This ordinance shall be known and may be cited as the Lone
Pine Fire Protection District Emergency Response Services Tax
Ordinance. The tax imposed by this ordinance is applicable to real
property located within the boundaries of the Lone Pine Fire
Protection District (hereinafter referred to as the District).
SECTION II. AUTHORITY.
The District is a duly formed and organized Fire Protection District
under the California Fire Protection District Law of 1987 Health and
Safety Code Sections 13800-13970. Health and Safety Code Section
13801 provides that the provision of emergency response services,
ambulance services, fire protection and other services related to
the protection of lives is critical to the public peace, health, and
safety of the state. Health and Safety Code Sections 13861and
13862, empower District to provide emergency medical, ambulance,
and other services to protect lives within its established boundaries.
Where the revenues available to the District are inadequate to meet
the costs of providing emergency response services, the District is
authorized by Health and Safety Code Sections 13910, 13911 and
13913 to impose a special non ad valorem property tax pursuant
to Government Code Sections 50075 and 50077, in accordance with
Articles 13A Section 4 and 13C Section 2 of the California Constitution
and Sections 53722 and 53724 of the Government Code. Such taxes
are authorized by Health and Safety Code section 13899 to be
collected at the same time and in the same manner as other county
taxes.
SECTION III. PURPOSE.
This ordinance proposes and, with voter approval, imposes a
special non ad valorem tax upon real property within the District for
the purpose of funding g emergency medical, ambulance services,
and other services to protect the lives within the District. This
ordinance shall be interpreted liberally to accomplish this purpose.
SECTION IV. VOTER APPROVAL REQUIRED.
As required by law, the District shall call for a special election
to be conducted within the boundaries of the District on November
4, 2014, for the purpose of submitting the special tax proposed by
this ordinance to all of the electors within the District. Such election
shall be consolidated with the UDEL election held on that date and
will be conducted in conformity with the Uniform District Election
Law (Election Code 10500 et. seq.) and constitutional and statutory
voting requirements. The Special tax proposed by this ordinance
shall be imposed only if it is approved by two-thirds (2/3) of the
voters voting upon such tax in such election.
SECTION V. SPECIAL EMERGENCY RESPONSE SERVICES.
Subject to the requirement for voter approval set forth in Section
IV above, the District hereby imposes a Special Emergency Response
Services Tax upon real property within the District as follows:
A. Purpose of Tax. The Special Emergency Response
Services Tax will enable the District to continue and improve and
fund emergency services. The revenues from the Special Emergency
Response Services Tax shall be used by the District for the purpose
of obtaining, furnishing, operating, and maintaining ambulance
services, emergency medical equipment, for paying the salaries and
benefits to emergency response personnel, and for such other
emergency response and prevention expenses of the District. The
Revenues from this tax shall not be used for any other purpose.
Without passage of this measure of the District may cease all
emergency response services and will cease ambulance services
on January 1, 2015.
B. Taxable Property. The Special Emergency Response
Services Tax is imposed upon all fee simple interests in real property
located within the Districts boundaries, except that the tax shall not
be imposed upon property belonging to the federal or state
government.
C. Annual Tax and Operative and Sunset Date. The Special
Emergency Response Services Tax will be imposed, and levied
annually each fiscal year, commencing July 1, 2015 and ending June
30, 2025.
D. Basis and Amount of Annual Tax. The Special Emergency
Response Services Tax is imposed and levied annually upon each
secured parcel of taxable property within the District at $100 per
parcel.
E. Tax Collection. The Special Emergency Response
Services Tax shall be collected on behalf of the District by the County
of Inyo in the same manner and subject to the same penalties,
interest, fees, and costs as, or with, other charges and taxes fixed
and collected by the County. The County may deduct its reasonable
costs incurred for such service before remitting the balance of the
tax proceeds to the District.
F. Creation of Fund: The funds attributable to this tax shall
be placed in a special account entitled Special Emergency Response
Services Tax Account and used exclusively for the purposes set
forth herein with annual reporting as required by Government Code
sections 50075.1 and 50075.3.
SECTION VI. SEVERABILITY.
If any section, subsection, sentence, clause, or phrase of this
ordinance is for any reason held to be invalid or unconstitutional by
a decision of any court of competent jurisdiction, such decision shall
not affect the validity of the remaining portion of this ordinance. The
Governing Board hereby declares that it would have passed this
ordinance, and each and every section, subsection, sentence,
clause, or phrase not declared invalid or unconstitutional without
regard to whether any portion of this ordinance would be subsequently
declared invalid or unconstitutional.
SECTION VII. EFFECTIVE DATE.
This ordinance provides for the imposition and levying of a special
non ad valorem property tax within the District, and shall only become
effective upon approval by two thirds (2/3) of the voters voting upon
such tax at a consolidated statewide election held on November 4,
2014. This Ordinance shall become effective immediately upon such
approval.
SECTION VIII. PUBLICATION.
Before the expiration of fifteen (15) days from the adoption thereof,
this Ordinance shall be published once in the newspaper of general
circulation printed and published in the County of Inyo, State of
California. The Districts Secretary is hereby instructed and ordered
to so publish the Ordinance together with the names of the Governing
Board members voting for and against the same.

Section 2. That language of the measure imposing and levying a
Special Emergency Response Services Tax upon fee
simple interest in real property within the District shall
be as set forth in Attachment A to this resolution.

consolidated with the UDEL election held on November
4, 2014, and will be conducted in conformity with the
Uniform District Election Law (Elections Code Sections
10500 et seq.) and constitutional and statutory voting
requirements.

ARGUMENT IN FAVOR OF MEASURE D
A yes vote on this issue will allow the Lone Pine Fire Department
to keep providing Ambulance Service to Area IV of the County of
lnyo. A no vote will cause ambulance service to stop on January
1, 2015.
While $100.00 per year may seem a lot, it really is a small
amount to pay if your life or the life of someone you love could
end, because you voted no. These funds will only go to provide
ambulance service and if for some reason the Lone Pine Fire
Department cannot provide the service the tax will end, it cannot
be transferred to someone else.
While the Lone Pine Fire District was established in 1964, it has
only been since 1994 that we have provided ambulance service.
As a result of this we do not receive any tax money for the
ambulance service and it needs to cover it's own expenses.
If this measure does not receive a 2/3 vote, it would be two
years before the next general election and by that time it will be
very hard to bring the service back.
This will be the first time that the Fire Department has gone to
the voter asking for any type of tax increase and we know that
you are there for us, just as we are here for you.
